      目的:观察术前集中应用多媒体互动性访视模式对于择期膝髋关节置换患者围手术期应激反应、医患满意度、人力成本等的影响,探讨集中多媒体术前访视模式的可行性。 方法:自2013年1月~2013年6月,共400例拟行膝髋关节置换患者随机分为集中访视组和对照组,分别于术前1日护理访视。集中访视组将手术患者集中,应用多媒体演示访视内容,集中互动回答问题和心理干预。对照组以一对一的口头交谈的方式告知相同内容。两组患者均于访视后及术后第二天进行焦虑评分、睡眠质量评分、患者及家属满意度评分、床位医生满意度评分;记录访视前、术前、术中、术后的血压和心率;统计两组应用人力成本。 结果:集中访视组的焦虑评分、睡眠质量评分、术中心率血压、患者及家属满意度评分、床位医生满意度评分与对照组有明显差异(P<0.05),而在术前术后的血压和心率方面差异不显著。同时,集中访视组明显节约了人力资本(P<0.01)。 结论:术前集中多媒体访视模式可以更加有效的减轻患者围手术期的生理心理应激反应,提高患者、家属、医生的满意度,节约人力资本,可操作性强、内容形式规范化,患者及家属参与积极,有利于提高术前访视效果。
英文摘要:
      Objective To observe the stress response, patients’ satisfaction and labor costs of centralized interactive multimedia inspection (CIMI), which be applied in the patients who were intended to perform total knee replacement. To discuss the feasibility of interactive multimedia inspection. Methods From Jan 2013 to Jun 2013, 400 patients who were planned to have total knee replacement were divided into two groups, the inspection group and the contrast group. The inspection group have all the patients watch the video of pre-operative multimedia inspection, answer the questions raised by patients as well as do the mental intervention to the patients. The contrast group make an oral introduction of the surgery. Both two groups have anxiety scale, sleeping scale, satisfactory scale; the blood pressure and heart rate were noted pre- and post-inspection as well as pre- and post-operation. The labor costs were noted for the CIMI group. Results The anxiety scale, sleeping scale, BP during the surgery, satisfactory scale of both patients and surgeons in two groups have significant statistical difference (P<0.05), but the difference is obvious in the pre- and post-operative BP and HR. Meanwhile, the CIMI group saved the labor costs, compared with contrast group (P<0.01). Conclusion The IMI can reduce the stress responses of patients during the perioperative. Improve the satisfactory of both patients and surgeons and save labor costs. The CIMI also possess the merits of high operability, standardized contents and form. The active involvement of patients and family members can improve the outcome of preoperative inspection.
