      目的 社区一专多能糖尿病管理护士开设糖尿病专科护理门诊，通过以点带面的个体化教育和社区四位一体的健康教育，提升社区居民对健康教育的认可度；通过规范管理患者，促进社区糖尿病及前期患者真正认识糖尿病并掌握糖尿病自我管理的能力。方法 由培训的糖尿病管理护士，建立糖尿病健康教育团队，设立糖尿病护理门诊，通过形式多样的个体化健康教育，提升本社区糖尿病患者的知识掌握率。结果 经过几个月的实践，已经初步形成了一定模式，并建立了一套完善的糖尿病教育和管理体系，患者健康教育前后6个月的糖尿病知识、正确服药、合理饮食、运动锻炼、血糖监测、空腹血糖和糖化血红蛋白等指标效果评价有统计学意义（P<0.05）。结论 社区一专多能糖尿病管理护士在护理门诊工作中使教育管理的糖尿病人能进行自我管理、规范管理血糖，养成良好的行为方式，提升管理的达标率；是护理模式的创新、优质护理的延伸。
英文摘要:
      [Abstract] : Objective In the management of diabetes community nurses opened diabetes outpatient nursing, health education through to individual education and community integration of the four community residents, improve the health education acceptance; through standardized management of patients, promote community diabetes and patients with pre diabetes truly understand and master the ability of diabetes self-management.Methods diabetes management of nurses by training, the establishment of diabetes health education team, the establishment of outpatient diabetes care, health education through various forms of individual, enhance the knowledge rate of diabetic patients in community. Results after months of practice, has initially formed a certain pattern, and the establishment of a comprehensive diabetes education and management system, there is statistical significance evaluation 6 months of diabetes knowledge, correct medication, diet, exercise, blood glucose monitoring, fasting blood glucose and glycosylated hemoglobin before and after the health education of patients (P<0.05). Conclusion community nurses in nursing management of diabetes in the clinical work of the diabetes education management can carry out self management and standardize the management of blood glucose, develop good behavior, enhance the management of the standard rate; is the extension of innovation, high quality nursing care model.
