| 目的 了解上海地区部分爱婴医院的孕妇孕晚期纯母乳喂养意向的现状，并分析其影响因素。方法 对2015年10月—2016年1月在上海8所爱婴医院门诊行产检的孕晚期孕妇进行问卷调查，调查内容包括其孕晚期一般情况、母乳喂养知识、婴儿喂养态度、母乳喂养自我效能、周围人对其母乳喂养的支持情况以及预期的喂养方式。结果 82.3%的孕妇在孕晚期具有纯母乳喂养意向。孕晚期纯母乳喂养意向与计划产后开始工作的时间、计划宝宝出生后主要照顾者、孕晚期母乳喂养态度、孕晚期自我效能4个变量有关；计划产后越晚开始工作的产妇其纯母乳喂养意向越强，计划产后6个月以后工作的产妇其孕晚期纯母乳喂养意向是计划产后6个月以内工作的产妇的3.79倍。计划由夫妻双方和长辈共同照顾宝宝的与计划仅夫妻双方照顾宝宝的相比，宝宝出生后计划仅由夫妻双方照顾的孕妇孕晚期纯母乳喂养意向越强。孕晚期研究对象母乳喂养态度得分越高，孕晚期纯母乳喂养意向越强。孕晚期研究对象自我效能得分越高，孕晚期纯母乳喂养意向越强。结论 上海地区的孕晚期妇女具有较积极的纯母乳喂养意向，但仍有部分孕妇不打算产后进行母乳喂养。医护人员应加强产前教育，提供母乳喂养知识和技能指导，提高孕妇的母乳喂养自我效能，培养其正向的婴儿喂养态度，在孕晚期帮助孕妇建立纯母乳喂养意向。 |

| Objective: To investigate the current situation of exclusive breastfeeding intention in third trimester of pregnant women in Shanghai part of baby-friendly hospital, and to analyze its influencing factors.Methods: according to the purpose of research,This study was carried out in eight baby-friendly hospitals of Shanghai, From October 2015 to January 2016, pregnant women were recruited at the outpatient department after they were qualified for the study. They were required to fill in the questionnaires including general information of third trimester,breastfeeding support received during pregnancy, knowledge、attitudes and self-efficacy of breastfeeding, and their intention to breastfeeding. Results: The intention of exclusive breastfeeding was 82.3% during the third trimester ,which were related to the time after the postpartum start work、the primary caregiver after the baby's birth、the attitude of breastfeeding at the end of pregnancy and self-efficacy in late pregnancy .The later the puerpera who plan to start work after delivery are more motivated to exclusive breastfeeding.Compared with the puerpera who plan to start work after delivery within 6 months ,the puerpera who start to work after 6 months are 3.79 times.In contrast to the plan of the spouses and the elders to take care of baby,planned by only couples to take care of baby,having a more active awareness to exclusive breastfeeding .the higher the breastfeeding attitude score,the stronger the intention of exclusive breastfeeding in third trimester. the higher the self-efficacy score of the third trimester,the stronger the intention of exclusive breastfeeding . Conclusion: Pregnant women in Shanghai area have positive intention to exclusive breastfeeding, but some pregnant women still do not plan to breastfeeding after giving birth. Medical staff should enhance prenatal education, provide breastfeeding knowledge and skills, improve breastfeeding self-efficacy of pregnant women, cultivate positive attitude of infant feeding,developing the intention of exclusive breastfeeding for pregnant women in late pregnancy . |
