      目的：积极应对上海人口老龄化, 完善社会养老服务体系,保障高龄老人得到居家医疗护理服务。方法：从2013年8月起，根据“上海市高龄老人医疗护理计划试点工作的通知”要求，莘庄镇纳入上海市六个试点街镇之一，由社区卫生服务中心医护人员对符合条件的户籍、年龄80周岁以上、参加本市城镇职工基本医疗保险的老人根据需求提供上门评估，根据评估结果，由护理站为老人提供居家医疗护理服务。结果：通过近三年的试点，有396位高龄老人提出了服务申请，经评估目前有181位老人在享受居家医疗护理服务，主要以基础护理项目为主，临床护理项目相对开展偏少。结论：试点工作虽然取得了一定的成效，但高龄老人护理人才队伍整体业务能力不高，急需提高。
英文摘要:
      Respond to the aging of the population of Shanghai actively, improve the social pension service system, protect the elderly home health care services. Method: Since August 2013, according to requirement of " Notice of the pilot of Shanghai elderly medical care plan ",Xinzhuang town is one of the six pilot towns in Shanghai, the community health service center staff provide on-site assessment according to the elders' requirements who conform to the conditions of the household registration, more than 80 years old, took part in the basic medical insurance for urban workers in the city. The nursing station provide home health care service for elders according to the results of the assessment. Result: During the pilot in nearly three years, 396 elders filed petitions for the service. After assessment ,there is 181 elders is enjoying home health care services which is mainly primary care project. Clinical nursing program is relatively less. Conclusion: Pilot work has made certain achievements, but the overall business ability of the elderly nursing personnel team is not high, need to improve urgently.
