| 【】：目的 研究早期康复的集束化护理在重型颅脑损伤患者中的应用效果。方法 选择2015年10月至2016年4月收治的重型颅脑损伤患者52例为对照组,采用神经外科常规的康复护理；选择2016年5月至2016年11月收治的重型颅脑损伤患者46例为集束化组,集束化管理方法包括保持肢体及关节的功能位置、关节被动运动、功能性电刺激、四肢循环治疗、促醒疗法、主动运动训练、吞咽功能障碍训练、自理能力训练、语言和智能训练、心理社会的调适等一系列有循证支持的联合护理措施,两个月后比较两组患者GCS评分、肌力分级以及ADL评分的差异。结果 集束化组康复护理2个月后GCS评分、肌力分级评定和ADL评分显著高于对照组,差异有统计学意义(P<0.01)。结论 早期康复护理的集束化管理可有效改善神经及肢体运动功能,提高重型颅脑损伤患者的日常生活能力,提高患者的生存质量和预后。 |

| Objective: To study the application effects of bundle nursing of early rehabilitation on patients with severe craniocerebral injury. Method: 52 patients with severe craniocerebral injury accepted from November 2014 to May 2015 were selected as control group and treated by routine rehabilitation nursing in neurosurgery; 46 patients with severe craniocerebral injury accepted from June 2015 to December 2015 were selected as the bundle group. The bundle methods include maintaining the functional location of limbs and joints, passive movement of joints, functional electrophotoluminescence, limb circle treatment, wake-promoting therapy, active movement training, swallowing dysfunction training, self-care training, language and intelligence training, psychosocial adjustment and other evidence-based combined nursing measures. Two months later, GCS grade, muscle strength grading, and ADL scoring of patients were compared between two groups. Results: After 2 months, GCS grade, muscle strength grading, and ADL grade of patients in bundle group were significantly higher than those in control group with difference of statistical significance (P<0.01). Conclusion: The bundle strategies of early rehabilitation nursing can effectively improve nerves and limb movement function, enhance the daily living ability of patients with severe craniocerebral injury, and increase the living quality and prognosis of patients. |
