      [摘要]目的 比较两种穿刺手法在老年患者动脉血气分析桡动脉穿刺采血的应用效果，探讨临床动脉采血的最佳穿刺方法。方法 将98例患者按照入院单双日随机分为对照组（n=50）和观察组(n=48)，对照组采用传统垂直进针法；观察组采用手掌背曲、桡动脉“一”字痕加桡动脉按压呈300～450进针，比较两组患者一次穿刺成功率，皮下淤斑发生情况及疼痛程度。 结果 观察组一次穿刺成功率明显高于对照组（x2=4.845，P=0.028）；观察组皮下淤斑发生率明显低于对照组（x2=4.400，P=0.036）；观察组穿刺过程中疼痛评分高级别例数明显低于对照组（Z=-2.111，P=0.035）。 结论 对行动脉血气分析的老年患者采用手掌背曲、桡动脉“一”字痕加桡动脉按压穿刺法，可显著提高一次穿刺成功率,明显减轻患者痛苦，值得临床推广应用。

      Abstract: Objective To compare the effect of two kinds of puncture technique in the arterial blood gas analysis of the elderly patients with radial artery puncture, and to explore the best way to get the blood. Methods 98 patients according to admission days were randomly divided into control group (n=50) and observation group (n=48), the control group used the traditional vertical needling; the observation group with palm dorsiflexion, radial artery "one" word mark and the radial artery was 300 ~ 450 pressing into the needle, the puncture were compared between the two groups the success rate and the incidence of pain and subcutaneous ecchymosis. Results the successful rate of puncture of observation group was significantly higher than the control group (x2=4.845, P=0.028); observation group subcutaneous ecchymosis was significantly lower than the control group (x2=4.400, P=0.036); the observation group during puncture pain score level were obviously lower than the control group (Z=-2.111, P=0.035). Conclusion the palm dorsiflexion, radial artery "one" word mark and press the radial artery puncture on the elderly patients with arterial blood gas analysis, can significantly increase the success rate of puncture, alleviate pain of patients, is worthy of clinical application.
