      目的 回顾性调查动态血压测量失败的原因，分析相关因素，并探索提高测量成功率可采取的措施。方法 收集2016年1月至5月瑞金医院动态血压室187例受检者的动态血压监测报告，从测量失败事件代码调查患者测量失败的因素，得出动态血压测量失败原因构成比。结果 测量失败的原因分两类：人为因素和仪器因素。人为因素中构成比最高的是受检者的过度活动或心律失常，占28.6%。仪器因素中，仪器的气泵产生问题最常见，占33.54%。过度活动或心律失常在不同生活自理能力和不同就医情况中存在统计学差异（P<0.05）。病理因素在不同性别患者中存在统计学差异（P<0.05），患者依从性不良在不同性别、不同生活自理能力、不同就医情况均不具有统计学差异(P>0.05)。结论 患者的过度活动或心律失常是动态血压监测失败的主要原因，提高健康宣教，有助于提高动态血压测量的有效性。
英文摘要:
      Objective To analyze the reasons for the failure in ambulatory blood pressure monitoring (ABPM) and to take corresponding measures. Methods Data of 187 patients who underwent ABPM in Ruijin Hospital from January 2016 to May 2016 were retrospectively studied for the failure report of event code. The data were statistically analyzed and the causes constituents ratio of event code was obtained. Results There were two reasons for the failure of measurement: human factors and instrumental factors. The leading cause in human factors was excess movement artifact or heart rate arrhythmia. The problem of blood pressure pump was the most common problem in instrumental factor. The differences of the excess movement artifact or heart rate arrhythmia with different activities of daily living and medical behavior were statistically significant (P<0.05). The difference of the pathological factors in the gender was statistically significant(P<0.05). The differences of the compliableness of behaviors following doctor’s advice with different gender, activities of daily living and medical behavior were not statistically significant (P>0.05). Conclusions The main causes of failure in ABPM are excess movement artifact or heart rate arrhythmia. We should pay attention to strengthen health education. Health education is helpful to improve the validity in ABPM.
