      Abstract: Objective to investigate the reasonsof diabetes mellitus patients losing visit in Cao Yang community, and to give follow-up intervention to analyze the effect of intervention. Methods 150 patients with diabetes mellitus losing visit in Cao Yang community clinic were selected as the research subjects. The causes of the patients were investigated by questionnaire.The patients were followed up for 12 months. The health knowledge and blood sugar level were compared before and after the intervention. Results the main reasons oflosing visit in the diabetes mellitus patients were insufficient attention to follow-up, physical reasons and cognitive errors. Through the 12 month follow-up intervention, the diet, medication, exercise, self-monitoring and other aspects of health knowledge of the patients were better than those before the intervention, the difference was statistically significant (P<0.05). The fasting blood glucose and postprandial 2h blood glucose of patients were lower than those before the intervention, the difference was statistically significant (P<0.05). Conclusions There are many reasons for diabetes patients losing visit. It is necessary to strengthen the follow-up intervention for patients, and give the patients subjective motivation to promote behavior change.
