      【】目的：胰岛素是妊娠糖尿病患者治疗的常规药物,但注射胰岛素引起的皮下脂肪增生往往不被医护人员及患者重视。本研究分析注射胰岛素的妊娠糖尿病患者发生皮下脂肪增生的现状及相关因素,为更合理更安全使用胰岛素提供依据。方法：入组使用人胰岛素治疗的妊娠糖尿病患者共145例,检查皮下脂肪增生的发生情况,并使用调查问卷收集受试者注射胰岛素的习惯。统计分析皮下脂肪增生的发生率,比较发生皮下脂肪增生患者的生化特征,并利用回归分析得出皮下脂肪增生的相关危险因素。结果：145例受试者中,共52人(35.9%)发生皮下脂肪增生。无皮下脂肪增生者相对脂肪增生者孕周更短(33.4±2.5 vs 36.8±3.6周,P=0.040),糖化血红蛋白更低(6.1±1.0 vs 6.4±0.8%,P=0.023),每日胰岛素总量更少(21.7±9.1 vs 28.1±8.8单位,P=0.002)。回归分析显示,轮替注射部位(OR=0.36,95%CI 0.136-0.956,P=0.04)及不重复使用针头(OR=0.105,95%CI 0.015-0.727,P=0.022)均为皮下脂肪增生发生的独立影响因素。结论：皮下脂肪增生在使用胰岛素治疗的妊娠糖尿病患者中并不罕见,发生皮下脂肪增生可能影响血糖控制。我们应该加强对此类患者的宣教和护理。

      【】Objective：Insulin is the regular medicine in the treatment of gestational diabetes.But lipohypertrophy caused by insulin injection is often ignored by patients and medical staffs. The aim of our study is to analyze the relationship between insulin injection and lipohypertrophy in patients with gestational diabetes. Methods：145 gestational diabetic patients with insulin injection was enrolled. Lipohypertrophy was examined and questionnaires were collected. Statistical analysis was performed between the clinical characteristics and lipohypertrophy. Results：52 people(35.9%)suffered from lipohypertrophy. Compared with these people, people without lipohypertrophy showed shorter gestational weeks(33.4±2.5 vs 36.8±3.6w,P=0.040), lower HbA1c levels (6.1±1.0 vs 6.4±0.8%,P=0.023) and less insulin (21.7±9.1 vs 28.1±8.8u, P=0.002). Logistic regression showed that regularly changing injection location (OR=0.36, 95%CI 0.136-0.956,P=0.04) and not using needles repeatedly (OR=0.105, 95%CI 0.015-0.727,P=0.022) are independent factors of lipohypertrophy. Conclusion：Lipohypertrophy is common in patients with gestational diabetes mellitus and is correlated with worse glucose control.
