      Objective To compare the PICC cannulation success rate and safety of two mathods by intracavitary electrocardiogram (IC ECG) guidance and conventional anatomical landmarks (CAL) in patients prospectively and randomly. Methods Patients who satisfied the inclusion criteria for PICC in our hospital were randomized to the IC ECG and CAL groups and their PICC placement were operated with IC ECG or CAL methods respectively. The post-procedural confirmation of catheter tip was determined by orthophoria chest radiograph (OCR). Intubation success rate and safety were primary measures. Results A total of 152 patients were randomized to the IC ECG (n =76) and CAL (n =76) groups respectively. The overall cannulation success rate of IC ECG was superior significantly than CAL(98.7% vs. 82%,P <0.01). OCR showed that all tips of PICCs guided with IC ECG were located in the optimal position. Regarding the cannulated veins and sites of catheters, and mean procedure time, there were no difference between two groups (P >0.05). There were no adverse events related to the catheter cannulating. Conclusions PICC guided with IC ECG is simple ,safe and reliable. When catheter cannulation was completed with guidance of IC ECG, the post-procedural OCR is unnecessary.
