| 目的 探讨改良安置胃管法在口腔颌面肿瘤术后患者的一次性成功率及患者舒适度的应用效果。方法 选择2017年1月—2017年10月我科收治口腔恶性肿瘤术后患者300例,按1、2、3编号随机分组,即常规A 组(用传统方法安置胃管)、改良B组(在插管前3分钟用呋麻液3滴滴于鼻孔,以收缩鼻咽粘膜,5分钟后再用l%丁卡因喷于鼻孔及口咽部、改良C组(经口腔插入),观察三组安置胃管的置管不良反应、置管时间及一次性成功率、患者舒适度情况。结果 改良B组、C组在置管不良反应次数、置管时间均少于A组,一次性置管率也高于A组,均有统计学差异(P值＜0.05),C组的的舒适度低于A 组、B组,有明显的统计学意义((P值＜0.05)。结论 将胃管经口腔插入及1%呋麻液滴鼻,l%丁卡因喷于双鼻孔及口咽部这两种方法均可减少置管不良反应及置管时间,提高口腔颌面肿瘤术后患者一次性插管率,由于l%丁卡因喷于双鼻孔及口咽部方法不适宜用于易感体质、年老体弱患者,口腔插入组舒适度虽然低于鼻腔置入组,因其操作简单易行,有药物过敏史、鼻中隔偏曲、鼻腔狭窄、年老体弱等特殊情况的病人也值得考虑使用。 |

| Objective ：To discuss one-time successful rate and degree of comfort via improved gastric tube placement in postoperative patients with oral and maxillofacial tumors.Methods: Chose 300 hospitalized postoperative patients with oral and maxillofacial tumors from Jan.,2017 to Nov.,2017 and randomly divided them into group 1,2 and 3. That were regular group A (placing gastric tube in regular way),improved group B (applied 3 drops of ephedrine and furacilin nasal drops 3 minutes before intubation to the nostril and l % tetracaine to nostril and mouth pharynx after 5 minutes) and improved group C (inserted via oral cavity), observed adverse reactions, placing time, one-time successful rate and degree of comfort in three groups of gastric tube placement.Results: Improved group B and C showed fewer times of adverse reactions and less placing time and more one-time successful rate than regular group A with statistic difference(p<0.05), and degree of comfort in group C is obviously lower than group A and B with significant statistic difference(p<0.05).Conclusions: Two methods, inserting via oral cavity and applying ephedrine and furacilin nasal drops 3 minutes before to the nostril and l % tetracaine to nostril and mouth pharynx, both help to reduce times of adverse reactions and placing time as well as one-time successful rate. Because of lower degree of comfort by inserting via oral cavity than via nasal cavity together with its simple operation, it is worth using widely on patients with a history of drug allergy, deflection of nasal septum, nasal cavity stenosis, and also frail elderly patients. #$NLKeywords:gastic tube inserting method;oral and maxillofacial tumor; nursing; |
