| 目的 了解初诊登记建卡时孕妇焦虑症状的发生状况并分析其相关影响因素。方法 在孕妇初诊登记时采用广泛性焦虑量表(GAD-7)筛查孕妇焦虑状况,采用非条件logistic回归分析焦虑症状的相关危险因素。结果 共纳入1423名孕妇,孕16周以内,以GAD-7量表10分为分界值,焦虑症状阳性检出率为4.2%。6个因素进入logistic回归模型：目前对居住条件和经济条件不满意(OR：14.669,95％CI：2.365~90.992)、孕期经常对他人产生不满情绪(OR：14.504,95％CI：5.263~39.969)、近3个月健康状况不佳(OR：3.591,95％CI：1.604~8.037)、经常过分担心胎儿健康状况(OR：3.332,95％CI：1.315~8.388)、有时个性敏感(OR：3.240,95％CI：1.500~6.999)、经常过分担心异常孕期检查结果(OR：2.898,95％CI：1.048~8.012)。结论 产科医务人员需要及早关注孕妇的焦虑状态,尤其对于家庭经济/居住条件不良和个性敏感的孕妇。可预防为先,通过围孕期保健帮助孕妇做好生理和心理的双重准备,同时在初诊登记时及时通过健康教育传递正确知识,降低孕妇对自身和胎儿健康的不确定感。 |

| Objective To obtain the prevalence of anxiety in early pregnancy and analyze the relevant influencing factors. Methods 7-item Generalized Anxiety Disorder Scale(GAD-7)was used to screen the anxiety symptoms during pregnancy .Logistic regression analysis was applied to analyze the related factors of antepartum anxiety symptoms. Results A total of 1,423 pregnant women within 16 weeks were included. A cut-off score of 10 or more of the GAD-7 scale indicates a positive screen, while 4.2% had antenatal anxiety symptoms. Six risk factors were included in the logistic regression analysis, showing that antenatal anxiety symptom was associated with dissatisfaction with the current living and economic conditions, common dissatisfaction with others during pregnancy, poor health in the past 3 months, too much worry about fetal health, sensitive personality, too much worry about abnormal pregnancy test results. Conclusion Medical staff need to pay early attention to antenatal anxiety symptoms, especially for pregnant women with poor economic or living conditions, as well as for whose personality is sensitive. As precaution is the first step, to help pregnant women to prepare themselves physically and mentally by perinatal care, as well as giving correct knowledge through health education in time in the first antenatal visit, in order to reduce pregnant women"s uncertainty about their own and fetal health. |
