      摘要 [目的]观察V-VST(容积-粘度吞咽测试)对脑卒中后吞咽障碍合并焦虑抑郁患者的影响。[方法]选取卒中后吞咽障碍合并焦虑抑郁患者90 例为干预对象,并随机分为观察组和对照组各45例。对照组采用神经内科常规治疗护理,观察组在对照组的基础上实施V-VST,对患者进行吞咽功能筛查,每周测试1次,连续4周,根据筛查结果采取针对性的饮食护理。观察干预后两组吞咽功能改善情况、干预前后两组汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分变化。[结果]两组干预后,观察组及对照组有效率分别为88.9%、66.7%,观察组较对照组改善更明显(P<0.05),差异具有统计学意义;两组干预后HAMD、HAMA评分均较干预前明显改善(P<0.05),观察组上述评分指标较对照组改善更明显(P<0.05),差异具有统计学意义。[结论]V-VST筛查患者进食食物的类型及一口进食量,并为其采取针对性的饮食护理,可以促进患者吞咽功能的恢复,减轻卒中患者焦虑抑郁情绪,改善患者生活质量。

      Abstract [Objective] To observe the influence of V-VST (Volume-Viscosity Swallowing Test) on dysphagia patients with anxiety and depression after cerebral apoplexy. [Method] 90 dysphagia patients with anxiety and depression after apoplexy were chosen as the intervention targets and randomly divided into the observation group and control group, 45 patients for each group. The control group was given routine treatment and nursing of neurology department, while the observation group was given V-VST on the basis of the control group to screen the swallowing function. The test was conducted once a week for consecutive four weeks. The target dietary nursing was adopted according to the screening results. The improvement of swallowing functions of the two groups after intervention, HAMD and HAMA score changes of the two groups before and after intervention were observed. [Result] After intervention, the effective rates of the observation group and the control group were 88.9% and 66.7%. The improvement of the observation group was more apparent than that of the control group (P<0.05) and the differences was of statistical significance. HAMD and HAMA scores of the two groups after intervention were apparently improved than those before intervention (P<0.05) and the aforementioned score indexes of the observation group were improved more apparently than those of the control group (P<0.05). The difference was of statistical significance. [Conclusion] V-VST screens the food types and mouthful food intake of patients and provides targeting dietary nursing, which can promote the restoration of patients’ swallowing function, reduce the anxiety and depression of apoplexy patients and improve the patients’ life quality.
