| 目的 探讨全程个案管理联合微信群对糖尿病患者的疾病认知状况、自我管理行为、血糖代谢及生存质量的干预效果。方法 2018年6月～2019年9月将某三级甲等医院内分泌病房的100例患者随机分为两组，对照组给予常规糖尿病护理，实验组给予为期6个月的全程个案管理联合微信群干预，使个案管理联合微信群的模式延伸到社区。于患者干预前和出院6个月时对患者的疾病认知、自我管理行为及生存质量进行评估，并检测血糖代谢指标。结果 干预6个月后，两组自我管理行为中饮食控制、锻炼、血糖监测、高低血糖处理、足部护理得分差异有统计学意义（P＜0.05），而药物管理得分差异无统计学意义（P＞0.05）。且两组的疾病认知、血糖代谢水平及生存质量差异有统计学意义。结论 全程个案管理联合微信群可以提高糖尿病患者的疾病认知、自我管理能力及生存质量，有助于控制其血糖代谢水平，值得临床推广应用。 |

| Objective To explore the effect of the whole case management combined with WeChat group on the cognitive status, self-management behavior, blood glucose metabolism and quality of life of patients with diabetes mellitus. Methods ?From June 2018 to September 2019，the 100 patients in the endocrine ward of a third grade first-class hospital were randomly divided into two groups. Control group was given conventional diabetes care, experimental group was given the whole case management combined WeChat group intervention, the case management combined WeChat model extended to the community. The patients' disease cognition, self-management behavior and quality of life were evaluated before and after the intervention of the patient for 6 months, and the blood glucose metabolism index were detected. Results 6 months after intervention, two groups of self-management behavior of diet, exercise, blood sugar monitoring, high and low blood sugar processing, foot care score difference were statistically significant (P<0.05), and self management behavior of drug management was not significant (P>0.05).The differences in disease cognition, blood glucose metabolism and survival quality of the two groups were statistically significant. Conclusions? Whole case management combined with WeChat group can improve the disease cognition, self-management ability and quality of life of patients with diabetes mellitus, which can help control the blood glucose metabolism level, and is worthy of clinical application. |
