| 目的 深入了解使用胰岛素治疗的老年2型糖尿病患者健康促进行为形成的主要因素，为临床护
理干预提供依据。方法 采用目的抽样法，选取2021年8月至2022年1月在华中科技大学同济医学院
附属同济医院内分泌科住院的正在使用胰岛素治疗的 10例老年 2型糖尿病患者为研究对象。采用现
象学研究方法，对其进行半结构式访谈，并运用Colaizzi七步分析法对访谈资料进行分析、归纳并提炼主
题。结果 共提炼出2个主题、6个子主题。即使用胰岛素治疗的老年2型糖尿病患者健康促进行为成
因分为内源驱动力（疾病感知、健康相关控制感、健康信念）和外源驱动力（社会环境、家庭支持、信息支
持）。结论 据使用胰岛素治疗的老年2型糖尿病患者健康促进行为的成因，临床护理人员可通过强化
医患共享决策、增强心理资本、塑造科学的健康信念来强化内源驱动力，通过优化社会环境、给予家庭
支持、提供信息支持来加强外源驱动力，从而达到健康促进的目的。 |

| Objective To deeply understand the main reasons for adopting health promotion behaviors in
elderly patients with type 2 diabetes treated with insulin, providing a basis for clinical nursing
intervention. Methods A total of 10 elderly patients with type 2 diabetes who were hospitalized in the
Endocrinology Department, Tongji Hospital, Tongji Medical College of HUST from August 2021 to
January 2022 were selected by purposive sampling. They were investigated by semi-structured interviews
using a phenomenological approach, and their interview data was analyzed and concluded using Colaizzi''s
seven-step analysis method to extract themes as follows. Results A total of 2 themes and 6 sub-themes
were extracted: Endogenous driving forces (illness perception, health-related sense of control and health
belief) and exogenous driving forces (social environment, family support and information support) for
elderly patients with type 2 diabetes treated with insulin adopting health promotion behaviors.
Conclusion According to the reasons for adopting health promotion behavior in elderly patients with
type 2 diabetes treated with insulin, clinical nurses can enhance their endogenous driving forces by
strengthening doctor-patient shared decision-making, strengthening psychological capital and shaping
scientific health beliefs, and enhance their exogenous driving forces by optimizing social environment,
encouraging family support and providing information support, so as to promote their health. |
