      [摘要] 目的 针对孕产妇疾病救治、待产与分娩急需处置的特征,分析新型冠状病毒肺炎流行期间,基于气泡式管理策略在孕产妇分娩救治与疫情防控的效果。 方法 回顾性分析2022年3月1日至5月31日期间,沿海某三甲妇产科专科医院,产科采用多项管理措施,包括气泡式管理、隔离区域改造、再造孕产妇分娩处置流程、调配及培训人力资源、建立支持系统等。 结果 隔离区共接诊患者242例,其中顺产58例,产钳4例,剖宫产39例,流产6例,留院观察135例,其中4例核酸快速检测筛查出阳性,孕产妇急诊分级为Ⅰ和Ⅱ级,进入隔离产房小气泡分娩,均实施终末消杀,再次采样核酸检测阴性恢复使用,期间启用备用隔离产房;隔离区工作人员共有48位:医生12位、助产士6位、护士27位、医辅人员3位,期间未发生院内感染和职业暴露。 结论 采用气泡式管理、隔离区域改造、再造孕产妇分娩处置流程、调配及培训人力资源、建立支持系统等措施,可满足孕产妇疾病救治和疫情防控的需求,最终实现院内感染和职业暴露的双零目标。

      [Abstract] Objective According to the characteristics of maternal disease treatment and delivery in urgent need of treatment, we analyzed the effect of bubble management strategy on maternal delivery treatment and epidemic prevention and control of COVID-19 during the epidemic period Method Retrospective analysis was made on the period from March 1 to May 31, 2022, in third-level grade-A obstetrics and gynecology hospital in coastal China, multiple management measures were adopted including bubble strategy, reconstruction of isolated areas, reconstruction of maternal delivery disposal process, allocation and training of human resources, establishment of support systems, etc. Result A total of 242 patients were admitted to the isolated department in our hospital, including 58 eutocia patients, 4 obstetric forceps delivery patients, 39 cesarean delivery patients, 6 artificial abortion and 135 disease observation. A total of 4 patients with COVID-19 infection was considered as emergency grade I or grade II. After entering the isolation delivery room for small bubble delivery, the terminal disinfection and sterilization was carried out, and the nucleic acid test was negative again. During this period, the standby isolation delivery room was used A total of 12 doctors, 6 midwives, 27 nurses, 3 paramedics worked in the isolation area without nosocomial infection nor occupational exposure occurred. Conclusion According to the relative strategy in above methods, medical treatment system and epidemic prevention were realized. Emergency response plans of bubble rupture play a key role that nosocomial infection and occupational exposure can be avoided.
