| [摘 要] 目的 探究头部穴位按摩联合耳穴压豆对降低择期剖宫产患者术中焦虑及疼痛的效果。方法 采用便利抽样法，选择医院2022年7月-2023年6月80例择期剖宫产患者为研究对象，采用随机数字表法分为对照组与试验组，每组各40例，为避免组间沾染，将对照组患者安排在A病区，将试验组患者安排在B病区。对照组接受常规干预，试验组在对照组基础上给予头部穴位按摩联合耳穴压豆干预。观察两组入室时、入室后30min、手术时、手术开始30min、出室时血流动力学变化情况，比较两组干预前、麻醉前及术后6h心理状况[简版状态焦虑量表（SSAI）评估]、疼痛程度[疼痛强度简易描述量表（VRS）评估]及舒适度[Kolcaba舒适状况量表（GCQ）评估]。结果 试验组入室时、入室后30min、手术时、手术开始30min及出室时脉搏、收缩压、舒张压均低于对照组，血氧饱和度高于对照组，且两组脉搏、收缩压、舒张压及血氧饱和度的组别、时点、组别与时点交互作用比较，差异均有统计学意义（P＜0.05）；两组麻醉前、术后6h的SSAI评分均低于干预前，且试验组低于对照组，两组组别、时点、组别与时点交互作用比较，差异均有统计学意义（P＜0.05）；两组术后6h的VRS评分均高于干预前，但试验组低于对照组，且两组组别、时点、组别与时点交互作用比较，差异均有统计学意义（P＜0.05）；观察组麻醉前、术后6h的GCQ评分均高于对照组，且两组GCQ评分的组别、时点、组别与时点交互作用比较，差异有统计学意义（P＜0.05）。结论 头部穴位按摩联合耳穴压豆能维持择期剖宫产患者术中血流动力学稳定，降低术中及术后焦虑、疼痛程度，提高其舒适度。 |

| Objective To explore the effect of head acupoint massage combined with ear acupoint pressing beans on reducing anxiety and pain in patients undergoing elective caesarean section. Methods The convenient sampling method was used, 80 patients with caesarean section from July 2022 to June 2023 in the hospital were selected as the research objects. They were randomly divided into the control group and the test group, with 40 patients in each group. To avoid contamination between groups, the patients in the control group were arranged in Ward A and the patients in the test group were arranged in Ward B. The control group received routine intervention, while the test group received head point massage combined with ear point pressing bean intervention on the basis of the control group. The hemodynamic changes of the two groups were observed at the time of entering the operating room, 30 min after entering the operating room, during the operation, 30 min after the operation, and at the time of leaving the operating room. The psychological status [simple state anxiety inventory (SSAI) assessment], pain degree [pain intensity simple description scale (VRS) assessment] and comfort [Kolcaba comfort scale (GCQ) assessment] were compared between the two groups before intervention, before anesthesia and 6 h after operation. Results The pulse, systolic blood pressure, and diastolic blood pressure of the test group were lower than those of the control group at entry, 30 minutes after entry, surgery, 30 minutes at the beginning of surgery, and at exit, and the blood oxygen saturation was higher than that of the control group. Moreover, there were statistical significant differences in the group, time point, group, and time point interactions of pulse, systolic blood pressure, diastolic blood pressure, and blood oxygen saturation between the two groups (P<0.05). The SSAI scores of the two groups before anesthesia and 6 hours after surgery were lower than before intervention, and the test group was lower than the control group. The differences in the interaction between the two groups, time points, and time points were statistically significant (P<0.05). The VRS scores of both groups at 6 hours after surgery were higher than before intervention, but the test group was lower than the control group. The differences in the interaction between the two groups were statistically significant (P<0.05). The GCQ scores of the observation group before anesthesia and 6 hours after surgery were higher than those of the control group, and the differences in GCQ scores between the two groups were statistically significant (P<0.05). Conclusion Head acupoint massage combined with ear acupoint pressing bean can maintain the stability of hemodynamics during the operation of selected caesarean section patients, reduce anxiety and pain during and after the operation, and improve their comfort. |
