| 目的:通过Meta分析比较肩关节镜术后应用加压冷疗与传统冷疗的疗效差异。方法:系统检索中国知网、万方、中国科技期刊数据库、Pubmed、Embase、The Cochrane Library、Web of science、ClinicalTrails数据库中关于肩关节镜术后加压冷疗与传统冷疗的随机对照研究,采用Stata 15.1软件对数据进行分析,采用标准均数差(SMD)和95%可信区间表示结果。结果:共纳入7项随机对照研究,包括442名患者,其中接受加压冷疗与传统冷疗的患者分别为223名、219名。Meta分析结果显示,加压冷疗组术后 24h(SMD=-0.87) 和48h(SMD=-1.03)的疼痛视觉模拟评分均低于传统冷疗组;加压冷疗组术后24h和48h以外上髁为起点15cm处,20cm处,腋窝处的肿胀值和总体肿胀评分均低于传统冷疗组。结论:在肩关节镜术后患者中,加压冷疗相比于传统治疗方法,具有更好的缓解疼痛和肿胀的作用。 |

| Objective: The meta-analysis was performed to compare the difference in efficacy between the application of compression cold therapy and conventional cold therapy after shoulder arthroscopy.
Methods: Randomized controlled trials of postoperative compression cold therapy versus conventional cold therapy after shoulder arthroscopy were systematically searched in the CNKI, Wanfang, VIP, Pubmed, Embase, the Cochrane Library, Web of Science, and ClinicalTrails databases, and the data were analyzed using Stata 15.1 software. The standardized mean difference (SMD) and 95% confidence interval (CI) were used to express the results.
Results: Seven randomized controlled studies were included, encompassing 442 patients, The meta-analysis showed that the pain visual analogue scores in the pressurized cold therapy group were lower than those in the conventional cold therapy group both at 24h [SMD=-0.87, 95% CI (-1.29, -0.45), P<0.001] and at 48h postoperatively [SMD=-1.03, 95% CI ( -1.53, -0.52), P<0.001]. The overall swelling score [SMD=-0.86, 95%CI (-1.22, -0.50), P<0.001], the swelling value at the axilla [SMD=-1.19, 95%CI (-2.11, -0.27), P=0.011], the swelling value at 15cm [SMD=-0.61, 95%CI (-0.92, -0.29), P<0.001] and 20cm [SMD=-1.31, 95%CI (-2.46, -0.16), P=0.026] starting from the lateral epicondyle were lower in the pressurized cold therapy group than in the conventional cold therapy group at 24h postoperatively. The overall swelling score [SMD=-0.85, 95%CI (-1.44, -0.26), P=0.005], the swelling value of the axilla [SMD= -1.55, 95%CI (-2.95, -0.15), P=0.03], the swelling value of the lateral epicondyle at the starting point of 15cm [SMD=-0.81, 95%CI (-1.44, -0.18), P=0.012] and 20cm [SMD=-1.14, 95%CI (-1.57, -0.71), P<0.001] were also lower than those of the traditional cold therapy group at 48h after surgery.
Conclusion: In post-arthroscopic shoulder patients, compression cold therapy provides better pain and swelling relief compared to traditional treatments. |
