| 目的 探讨基于个案管理的中医乳腺护理门诊服务模式的应用效果。方法 采用便利抽样法,选取2022年6月至2023年6月期间在我院收治的60例乳腺癌术后淋巴水肿患者作为研究对象,按照入院时间先后顺序,将2022年6月—2022年12月入院的患者作为对照组,2023年1月—2023年6月入院的患者作为观察组,每组各30例。对照组运用常规护理(出院指导+中医乳腺护理门诊随访)；观察组在常规护理基础上运用基于个案管理的中医乳腺护理门诊服务模式,28天为1周期,共干预2个周期。于干预前、干预后采用患臂围周径测量差值、中医症候积分、自我护理能力量表及乳腺癌患者生命质量测定量表评价干预效果。结果 观察组的患臂围周径测量差值明显低于对照组(P＜0.05),观察组中医症候积分优于对照组(P＜0.05 ),观察组患者自我护理能力及生命质量总评分,显著高于对照组(P＜0.05)。结论 基于个案管理的中医乳腺护理门诊服务模式的干预,确保了患者能够从医院到出院回归家庭得到连续有效的专业护理,减轻了患者乳腺癌术后淋巴水肿情况,缓解了患者各类相关不适症状,提升了患者自我护理能力,改善了患者生命质量,因此值得推广和应用。 |

| Objective To explore the application effect of the Chinese medicine breast care outpatient service model based on case management. Methods Using the convenience sampling method,60 postoperative breast cancer lymphedema patients admitted to our hospital between June 2022 and June 2023 were selected as study subjects,and according to the order of admission time,patients admitted from June 2022 to December 2022 were treated as the control group,and patients admitted from January 2023 to June 2023 were treated as the observation group,with 30 cases in each group. In the control group, routine care (discharge counseling + TCM breast care outpatient follow-up) was applied;in the observation group,the TCM breast care outpatient service model based on case management was applied on the basis of routine care,with 28 days as a cycle and a total of 2 cycles of intervention.Before and after the intervention, the intervention effect was evaluated using the difference between the circumference of the affected arm,the TCM symptom score,the self-care ability scale and the quality of life scale for breast cancer patients. Results The difference in the measurement of the circumference of the affected arm in the observation group was significantly lower than that in the control group(P< 0.05),the TCM symptom score in the observation group was better than that in the control group(P<0.05),and the self-care ability and overall quality of life scores of the patients in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The case management-based TCM outpatient breast care service model ensures that patients can receive continuous and effective professional care from hospital to home,reduces postoperative lymphoedema,relieves all kinds of discomfort,and improves patients" quality of life. |
