| 目的 分析慢性阻塞性肺疾病急性加重期(AECOPD)患者缓解呼吸困难恐惧的支持需求,为进一步筛选需求,在临床上更好地为患者制定服务方案提供理论依据。方法 本研究采取随机数字表法,纳入了2021年1月至2023年5月期间在医院治疗的337例AECOPD患者。采用Kano模型结合德尔菲专家咨询法设计AECOPD患者缓解呼吸困难恐惧的支持需求的调查问卷,采用Kano模型进行需求属性归类,结合Better-Worse系数及二维矩阵进行分析,确定信息需求属性,并通过需求筛选最敏感和需要改进的要素进行策略优化。结果 依据Kano模型对需求属性实施分类,并运用Better-Worse系数与二维矩阵进行综合性的分析评估,11个需求均落入优先改进区。其中期望需求有6个条目(54.55%),必备需求有3个条目(27.27%),魅力需求有2个条目(18.18%)。根据支持需求进行敏感性分析,其中灵敏度R最高为0.357,最低为0.021。其中条目4“药物使用”、条目11“紧急安全处理”、条目5“病情监测”是灵敏度最高的3个需求。结论 AECOPD患者缓解呼吸困难恐惧的支持需求中,灵敏度最高的3个需求分别为“药物使用”、“紧急安全处理”和“病情监测”。建议后续进一步加强患者对药物使用的相关知识教育,同时定期组织模拟演练以提高患者对呼吸困难恐惧的应对能力,并帮助患者建立病情监测记录表以定期评估病情。 |

| Objective: To analyze the support needs of individuals experiencing acute exacerbations of Chronic Obstructive Pulmonary Disease ( AECOPD ) to alleviate the fear of dyspnea, so as to provide a theoretical basis for further screening needs and better formulating service plans for patients in clinical practice. Methods: This study employed a random sampling method to include 337 AECOPD patients who were treated at the Hospital From January 2021 to September 2024. A survey questionnaire for support needs of AECOPD patients to alleviate fear of dyspnea was designed using the Kano model in combination with the Delphi expert consultation method. The Kano model was utilized for categorizing need attributes, and the Better-Worse coefficient and two-dimensional matrix were employed for analysis to determine the attributes of information needs. Furthermore, the most sensitive and improvable elements were screened through demand analysis for strategic optimization. Results: Based on the Kano model, the need attributes were classified and comprehensively analyzed and evaluated using the Better-Worse coefficient and a two-dimensional matrix, with all 11 needs falling into the priority improvement area. Among them, there were 6 entries (54.55%) for must-be requirements, 3 entries (27.27%) for one-dimensional (linear) requirements, and 2 entries (18.18%) for attractive requirements. A sensitivity analysis was conducted based on the must-be requirements, with the highest sensitivity value R being 0.357 and the lowest being 0.021. The top three demands with the highest sensitivity were item 4 "Medication Usage", item 11 "Emergency Safety Measures", and item 5 "Disease Monitoring". Conclusion: Among the support needs for AECOPD patients to alleviate the fear of dyspnea, the three needs with the highest sensitivity are "medication usage", "emergency safety measures", and "disease monitoring". It is recommended to further strengthen the education on medication-related knowledge for patients, while also regularly organizing simulation exercises to enhance patients' ability to cope with the fear of dyspnea. Additionally, helping patients establish a disease monitoring record sheet for regular assessment of their condition is advised. |
