| 目的 探究信息化5P医学模式在原发性开角型青光眼(Primary Open-Angle Glaucoma,POAG)患者中的临床应用效果。方法 本研究为一项前瞻性、单中心的队列研究,纳入111例POAG患者。基于所选管理模式不同,患者被分为信息化5P医学模式组(55例)和纸质化5P医学模式组(56例)。比较两组1年内随访率、眼压、疾病认知得分、用药依从性以及生活质量(包括全身健康状况及视力评分、活动方面的困难得分、对视力问题的反应评分和情绪问题)。结果 信息化5P医学模式组在第1个月(94.5% vs. 53.6%)和第3个月(90.9% vs. 53.6%)的随访率显著高于纸质化5P医学模式(P均<0.001),但在第6个月和第12个月,两组随访率无明显差异(P均>0.05)。在各个随访时间点,两组眼压值较干预前均显著下降,且信息化5P医学模式组眼压值较干预前变化量显著大于纸质化5P医学模式组(P均<0.05)。在第12个月,信息化5P医学模式组疾病认知得分、用药依从性得分、情绪问题得分显著高于纸质化5P医学模式组(P均<0.05),但在其他生存质量评估方面与纸质化5P医学模式组无差异(P均>0.05)。结论 信息化5P医学模式可有效提高患者早期随访依从性、降低眼压、提升疾病认知、增加用药依从性及改善情绪问题方面具有较好临床应用价值。 |

| Objective To explore the effectiveness of the informatics 5P medical model in patients with primary open-angle glaucoma (POAG). Methods This study was a prospective, single-center cohort study that enrolled 111 patients with high-tension POAG. Patients were divided into an informatics 5P medical model group (55 patients) and a paper-based 5P medical model group (56 patients), based on their chosen approach. The study compared the follow-up rates, intraocular pressure, disease cognition scores, medication adherence, and quality of life (included general health and vision scores, difficulty in activities scores, reaction to vision problems scores and emotional problems) between the two groups within one year. Results The follow-up rates of the informatics 5P medical model group at the first (94.5% vs. 53.6%) and third (90.9% vs. 53.6%) months were significantly higher than those of the paper-based 5P medical model group (all P<0.001), but there was no significant difference in follow-up rates between the two groups at the sixth and twelfth months (all P>0.05). At each follow-up time point, the intraocular pressure of both groups decreased significantly compared to baseline, and the change in intraocular pressure in the informatics 5P medical model group was significantly greater than that in the paper-based 5P medical model group (all P<0.05). At the twelfth month, the disease cognition scores, medication adherence scores, and emotional problem scores of the informatics 5P medical model group were significantly higher than those of the paper-based 5P medical model group (all P<0.05), but the other quality of life assessments were similar to those of the control group (all P>0.05). Conclusion The informatization 5P medical model can effectively improve patients with early follow -up compliance, reduce intraocular pressure, improve disease cognition, increase medication compliance, and improve emotional problems. |
