      目的 探究神经肌肉刺激器(NMES)在下肢骨折术后防治深静脉血栓形成的疗效。方法 采用随机数字表法将2023年7月—2024年6月某三甲医院血管外科收治的下肢骨折术后发生血栓并进行抗凝治疗的100例患者分为对照组和试验组,每组患者50例。对照组采用抗凝治疗+标准化护理流程+间歇充气压力泵(IPC)进行治疗,试验组采用抗凝治疗+标准化护理流程+神经肌肉刺激器(NMES)进行治疗。分别于患者抗凝治疗后第7天比较两组患者的凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(Fbg)、D-二聚体等凝血指标;采用彩色多普勒超声于抗凝治疗后7天测量两组患者股静脉及腘静脉的血流速度;采用视觉模拟量表(VAS)评价患者的疼痛度及患肢的肿胀程度;并于出院前对两组患者进行满意度调查。结果 经治疗后,试验组的平均凝血时间大于对照组,纤维蛋白原小于对照组,D-二聚体平均值小于对照组,差异有统计学意义(P<0.05);试验组的股静脉、腘静脉血流速度明显大于对照组,差异有统计学意义(P<0.05);试验组的疼痛评分、下肢周径差小于对照组,差异有统计学意义(P<0.05);出院前试验组的满意度(90%)明显高于对照组(74%),差异有统计学意义(P<0.05)。结论 神经肌肉刺激器相较间歇充气压力泵(IPC)在下肢骨折术后形成血栓的患者中能起到更好的疗效,患者满意度更高,值得临床进一步推广。

      Objective To investigate the efficacy of neuromuscular electrical stimulation (NMES) in preventing and treating deep vein thrombosis after lower limb fracture surgery. Method A total of 100 patients who developed thrombosis after lower limb fracture surgery and received anticoagulant therapy in the vascular surgery department of a tertiary hospital from July 2023 to June 2024 were selected as the study subjects, a random number table method was used and the patients were divided into a control group and an observation group, with 50 patients in each group. The control group was treated with anticoagulant therapy, standardized nursing procedures, and intermittent pneumatic compression(IPC), while the observation group was treated with anticoagulant therapy, standardized nursing procedures, and neuromuscular electrical stimulation (NMES).Compare the coagulation indicators such as thromboplastin time (APTT), prothrombin time (PT), fibrinogen (Fbg), and D-dimer between the two groups of patients on the 7th day after anticoagulant therapy; Color Doppler ultrasound was used to measure the blood flow velocity of the femoral vein and popliteal vein in two groups of patients 7 days after anticoagulant therapy; Use Visual Analog Scale (VAS) to evaluate the patient"s pain level and assess the degree of swelling in the affected limb; Conduct satisfaction surveys on both groups of patients before discharge. Results After treatment, the coagulation time of the observation group was higher than that of the control group, fibrinogen was lower than that of the control group, and the average D-dimer value was lower than that of the control group (P<0.05);The blood flow velocity of the femoral vein and popliteal vein in the observation group was significantly higher than that in the control group(P<0.05);The pain score and lower limb circumference difference of the observation group were smaller than those of the control group(P<0.05);The satisfaction rate of the observation group before discharge (90%) was significantly higher than that of the control group (74%)(P<0.05). Conclusion Compared with intermittent pneumatic compression(IPC), neuromuscular electrical stimulation can have better therapeutic effects and higher patient satisfaction in patients with thrombosis after lower limb fracture surgery, and is worthy of further clinical promotion.
