| 目的 观察中药泥敷治疗在缓解脑卒中后肩手综合征Ⅰ期患者疼痛及肿胀中的护理效果。方法 根据患者入院先后顺序将入组的204例脑卒中后肩手综合征Ⅰ期患者进行随机分配,即对照组及观察组各102例,对照组根据脑卒中患者护理常规进行护理(包括：康复护理+对症治疗)；观察组在对照组的基础上给予中药泥敷治疗,两组患者分别于干预前、干预后3天、干预后7天及干预后14天为靶点,采用视觉疼痛模拟量表、手部肿胀程度评分(参考排水法)及Fugl-Meyer运动功能量表对患者治疗前后关节疼痛程度、患侧部位肿胀程度及患侧上肢功能进行评定。结果 干预14天后观察组患者视觉模拟评分法(VAS)评分(1.68±1.75)较对照组干预前(5.36±1.27)明显降低,手部肿胀程度评分(82.58±8.26)较对照组干预前(171.25±8.78)显著降低,两组患者Fugl-Meyer运动量表(39.76±2.07)评分较对照组干预前(27.75±1.09)显著增高,两组比较差异均有统计学意义(P＜0.05)。结论 中药泥敷治疗可改善脑卒中后肩手综合征Ⅰ期患者患肢疼痛及肿胀症状,促进关节功能恢复,提高患者生活质量。 |

| Objective:To observe the nursing efficacy of herbal mud application in alleviating pain and swelling in patients with Stage I shoulder-hand syndrome after stroke.Methods:A total of 204 patients with Stage I shoulder-hand syndrome after stroke were randomly assigned to either a control group or an observation group,with 102 patients in each group,based on the order of their admission.The control group received routine nursing care for stroke patients (including rehabilitation nursing and symptomatic treatment),while the observation group received additional herbal mud application therapy. Both groups were assessed before the intervention, and at 3 days, 7 days,and 14 days after the intervention using the Visual Analog Scale (VAS) for pain, hand swelling score (based on the water displacement method), and the Fugl-Meyer Motor Function Scale to evaluate joint pain, swelling in the affected area, and upper limb function.Results:After 14 days of intervention, the VAS score in the observation group (1.68±1.75) was significantly lower than that of the control group before the intervention (5.36±1.27).The hand swelling score in the observation group (82.58±8.26) was also significantly lower than that of the control group before the intervention (171.25±8.78).The Fugl-Meyer Motor Function Scale score in the observation group (39.76±2.07)was significantly higher than that of the control group before the intervention (27.75±1.09).All differences between the two groups were statistically significant (P＜0.05).Conclusion:Herbal mud application can alleviate pain and swelling in the affected limbs of patients with Stage I shoulder-hand syndrome after stroke, promote the recovery of joint function, and improve the quality of life for patients. |
