| 目的 探究维持性血液透析患者恐惧疾病进展的潜在类别,并分析不同潜在类别的影响因素。方法 本研究采用多中心便利抽样法,选取2025年4月—2025年6月在6所三级甲等医院治疗的维持性血液透析患者作为研究对象。使用一般资料调查表、中文版恐惧疾病进展简化量表、中文版维持性血液透析患者自我效能量表、中文版心理弹性量表简版、中文版医学结果研究社会支持量表及透析症状指数量表进行调查。采用潜在剖面分析识别恐惧疾病进展的潜在亚组,结合单因素分析和有序多分类Logistic 回归模型探究其影响因素。结果 潜在剖面分析结果显示患者存在三个潜在类别:分别为“低风险-疾病适应组”(n=93,29.6%)、“中风险-疾病担忧组”(n=153,48.7%)、“高风险-功能失调组”(n=68,21.7%);其影响因素包括年龄、自理能力、家庭人均收入、透析龄、心理弹性水平、社会支持程度、透析症状负担水平(P<0.05)。结论 维持性血液透析患者恐惧疾病进展存在显著群体异质性,临床医护人员应根据不同亚组特征制定有针对性的干预策略,通过提高社会支持与心理弹性水平、减轻透析症状负担等方式,以降低其恐惧疾病进展水平。 |

| Objective To identify the potential categories of fear of progression (FoP) in patients undergoing maintenance hemodialysis (MHD) and to analyze the influencing factors for different latent categories.Methods This multi-center cross-sectional study employed a convenience sampling method to recruit MHD patients from six tertiary hospitals between April and June 2025. Data were collected using a general information questionnaire, the Chinese version of the Fear of Progression Questionnaire-Short Form (FoP-Q-SF), the Chinese version of the Perceived Kidney Disease Self-Management Scale (PKDSMS), the 10-item Connor-Davidson Resilience Scale (CD-RISC-10), the Chinese version of the Medical Outcomes Study Social Support Survey (MOS-SSS), and the Dialysis Symptom Index (DSI). Latent profile analysis (LPA) was used to identify potential subgroups of FoP. Univariate analysis and ordinal logistic regression were applied to explore the influencing factors.Results The LPA revealed three distinct latent categories of FoP: "Low-Risk Disease Adaptation Group" (n=93, 29.6%), "Medium-Risk Disease Concern Group" (n=153, 48.7%), and "High-Risk Dysfunctional Group" (n=68, 21.7%). Significant influencing factors identified included age, self-care ability, monthly household income per capita, dialysis vintage, psychological resilience, level of social support, and dialysis symptom burden (P < 0.05).Conclusion Significant heterogeneity exists in the fear of disease progression among MHD patients. Clinical healthcare providers should develop targeted intervention strategies based on the characteristics of different subgroups. This can be achieved by enhancing social support and psychological resilience, and alleviating dialysis symptom burden, thereby ultimately reducing patients'' fear of disease progression. |
