      目的:探讨阴道分娩的初产妇第二产程中以会阴中心腱紧张度标准控制胎头娩出速度的效果。方法:本次试验选取我院产科2024年11月-2025年4月收治的680例拟阴道分娩的初产妇进行研究,按照控制胎头娩出速度的不同标准划分为对照组(n=340)用力时阴道外口外露胎头<1cm标准控制胎头娩出速度与观察组(n=340)会阴中心腱紧张度标准控制胎头娩出速度,比较两组会阴裂伤率、新生儿窒息率、分娩出血量、会阴疼痛程度评分(NRS)。结果:观察组会阴裂伤率低于对照组(P<0.05);观察组1min新生儿窒息发生率较对照组更低(P<0.05);观察组产时、产后2h出血量及疼痛评分较对照组更低(P<0.05)。结论:阴道分娩的初产妇以会阴中心腱紧张度标准控制胎头娩出速度,可降低母婴不良结局发生率,减轻产妇疼痛感,实践价值较高。

      Objective: To investigate the effectiveness of perineal central tendon tension control in regulating fetal head descent during the second stage of vaginal delivery in primiparas. Methods: This study analyzed 680 primiparas scheduled for vaginal delivery at our hospital from November 2024 to April 2025. Participants were divided into two groups based on different fetal head descent control criteria: the control group (n=340) using the "external vaginal opening with fetal head <1cm" standard, and the observation group (n=340) applying perineal central tendon tension control. Comparative analysis was conducted on perineal laceration rate, neonatal asphyxia rate, perinatal hemorrhage volume, and perineal pain score (NRS). Results: The observation group demonstrated significantly lower perineal laceration rates (P<0.05) and neonatal asphyxia rates within 1 minute (P<0.05) compared to the control group. Additionally, the observation group showed reduced perinatal hemorrhage volume and lower pain scores during labor and 2 hours postpartum (P<0.05). Conclusion: Implementing perineal central tendon tension control for fetal head descent in vaginal delivery primiparas can effectively reduce adverse maternal-infant outcomes and alleviate maternal discomfort, demonstrating significant clinical value.
