      目的 探讨运用优质低蛋白结合a-酮酸饮食对维持性血液透析患者营养状况及电解质的影响及护理心得。 方法 将40名维持性血液透析患者随机分为低蛋白饮食组及正常蛋白饮食组。8周后低蛋白饮食组更改为正常蛋白摄入并继续随访8周。测定患者在治疗前,治疗4,8,16周时的血清生化值,记录其营养指标的变化。并由营养师制定个体化食谱。 结果 LP组患者的各种血清营养学指标以及身体营养指标较治疗前无明显改变。持续的心理护理使患者能更好地理解并配合饮食计划的实施,经过低蛋白饮食宣教及饮食反馈调整后,LP组患者热卡达标率较未干预前明显提高。结论 运用优质低蛋白结合a-酮酸饮食能维持透析患者营养状态的稳定,改善其高磷血症及酸中毒,在低蛋白饮食的治疗过程中添加心理护理和饮食宣教有助于提高治疗效果。

      Aim: To explore the effects of low dietary protein intake with keto acids diet on nutritional status and electrolyte metabolism and summary the nursing experience in maintenance hemodialysis patients. Methods: 40 MHD patients were randomly divided into a group of low-protein with keto acid diet (LP group) and normal protein diet group (NP group) for 8 weeks. After 8 weeks the LP group was shifted to NP for another 8 weeks. According to each patient’s BMI and eating habits, we designed and made recipes individually. Also, we recorded three-day diaries before and during the trial. Exsanguinate and determine the change of the value of serum biochemical. Results: In both groups, the nutritional index was unchanged during the trials. Serum phosphorus level was significantly decreased at the end of the first 8 weeks in LP group while CO2-CP was significantly increased. Conclusion: Using low dietary protein intake with keto acids diet can maintain the nutritional status in MHD patients and also significantly decrease the serum ameliorate hyperphosphatemia and metabolic acidosis.
