| 目的:探讨肺表面活性物质(PS)联合鼻塞式持续气道正压通气(NCPAP)治疗早产儿呼吸窘迫综合征(RDS)的临床疗效及护理对策。方法:选择90例早产RDS患儿,根据患儿家属是否同意使用PS分为观察组(n=46)和对照组(n=44),观察组给予PS联合NCPAP治疗,对照组不予PS处理,两组其他治疗及护理措施相同;两组患儿治疗前、治疗后第6小时、第24小时行血气分析,同时观察两组患者有创机械通气率、氧疗时间、住院时间、治愈率。结果:治疗前两组血气指标水平无显著差异(P>0.05),治疗后观察组血气指标较同期对照组有显著改善(P<0.05),有创机械通气率、氧疗时间、住院时间观察组显著低于对照组(P<0.05),治愈率则高于对照组(P<0.05)。结论:PS联合NCPAP是治疗早产儿RDS的有效方式,能显著改善肺功能,缩短病程,减少有创机械通气率,提高治愈率。正确、有效的护理措施的配合是治疗成功的重要保障。 |

| Objective: To explore the therapeutic effects and nursing of pulmonary surfactant (PS) combined with continuous positive airway pressure (CPAP) in treating premature infants with respiratory distress syndrome (RDS). Methods: A total of 90 premature infants with RDS were divided into treatment group (n=46) and control group (n=44) according to their parents agree to use PS or not. Both groups received routine treatment and nursing, while the patients of treatment group received CPAP combined with PS in addition. Blood gas analysis was detected at 0th, 6th and 24th hour in both groups. Group comparisons were made on the measures of invasive mechanical ventilation rate, time of receiving oxygen therapy, days of staying in the intensive care unit and curative ratio. Results: The results of blood gas analysis were not different in both groups before therapy(P>0.05). The results of blood gas analysis in treatment group were higher than those of control group after treatment (P<0.05). Invasive mechanical ventilation rate, time of receiving oxygen therapy and days of staying in the intensive care unit in treatment group were lower than those of control group (P< 0.05). Curative ratio in treatment group was higher than that of control group (P<0.05). Conclusions: Treating premature infants with RDS with PS combined with CPAP shows a remarkable curative effect. It can improve the lung function, shorten the course of disease, reduce invasive mechanical ventilation rate and improve the cure rate significantly. The correct and effective nursing measures are important guarantees for the success of the treatment. |
