      【摘要】 目的 探讨2型糖尿病患者胰岛素笔注射意愿的相关影响因素,为开展胰岛素笔注射技能教育提供理论依据。方法 采用方便抽样法选取上海市第五人民医院的200例2型糖尿病患者,对其进行规范化胰岛素笔注射技能教育后收集患者的一般资料、胰岛素笔注射意愿、临床生物学指标、教育学指标、社会心理学指标及社会经济学指标。结果 将200名2型糖尿病患者的继续注射胰岛素意愿按四分位法分为四组,第三四分位数为9,9分以上的患者占40%;Pearson相关分析结果显示:年龄(r =0.252,P<0.01)、职业(r =0.273,P<0.01)、婚姻(r =0.260,P<0.01)、病程(r =0.225,P<0.01)、并发症(r =0.358,P<0.01)、出院3个月后胰岛素注射技能得分(r =0.233,P<0.01)、两次胰岛素注射技能得分差(r =0.846,P<0.01)对患者继续胰岛素注射意愿有影响,呈正相关;出院时胰岛素注射技能得分(r =-0.595,P<0.01)、疼痛评分(r =-0.899,P<0.01)、注射部位有无红肿(r =-0.631,P<0.01)对患者继续使用胰岛素注射意愿有影响,呈负相关。线性回归分析显示:两次胰岛素注射技能得分差、疼痛评分、注射部位有无红肿是影响患者出院当日胰岛素注射技能得分的独立因素。结论 患者出院3个月后继续使用胰岛素注射意愿并不高,糖尿病专科护士在为患者制定并实施技能教育时可以充分考虑到相关影响因素,从而制定更为有效的教育手段,以提高2型糖尿病患者胰岛素治疗的意愿。

      【Abstract】Object To investigate the related factors for insulin pen injection willingness in patients with Type 2 diabetes, in order to conformation a theoretical basis for the education of insulin pen injection skills. Method 200 cases of type 2 diabetes were selected with convenience sampling method from the Fifth People's Hospital of Shanghai. Collecting the general data of them, and standardized education of insulin pen injection were provided for them. 3 months later, collecting the patients’ insulin pen injection willingness, clinical biological indicators, education indicators, social psychology indicators and socio-economic indicators. Result The Insulin pen injection willingness of 200 patients were divided into four groups with quartile method. The third quartile was 9, there are 40% patients got a score beyond 9. Pearson correlation analysis showed that age (r = 0.252, P <0.01), occupation (r = 0.273, P <0.01), marriage (r = 0.260, P <0.01), duration (r = 0.225, P <0.01), complications (r = 0.358, P <0.01), the scores of insulin injections skill 3 months after discharge (r = 0.233, P <0.01) and the change in scores of insulin injection skill (r = 0.846, P <0.01) has a positive correlation with patients’ continuing willingness in insulin injections. Score of insulin injection skill hospitalization (r = -0.595, P <0.01), pain score (r = -0.899, P <0.01), injection site redness and swelling (r = -0.631, P <0.01) has a negative correlation with patients’ continuing willingness in insulin injections. Linear regression analysis showed that the change in scores of insulin injection skill, pain score, and redness, swelling in the injection site is an independent factor which could affect patients’ insulin injection skill score hospitalization. Conclusion There is a weak willingness for the patients in continuing to use insulin pen injection 3 months after discharged. Diabetes specialist nurses should take consideration of all the relevant factors when formulate and implement skills-based education for the patients. In consequences of that, more effective means of education will be developed, and the willingness to use insulin pen injection will be improved in patients with type 2 diabetes.
