| 【】 目的 通过专科医护人员共同组成延续性服务团队,对DM患者实施专科宣教和沙龙、DM广播操干预护理服务。方法 对出院后的231名DM患者进行延续性电话服务和定期开展DM宣教讲座,来观察患者在自测血糖、血糖控制情况、足部护理、识别低血糖、使用胰岛素笔认识和实施状况。 结果 在231例DM患者中,能自己正确测定血糖(100%),空腹血糖控制在正常值209例(90.4%),糖化血红蛋白的测定控制在正常范围值203例(87.8%)；能正确知晓足部护理(97.5%)；能识别低血糖(98.5%)；有135例患者使用胰岛素笔,能正确使用胰岛素笔125例(92.6%)。结论 对DM患者实施延续服务,能提高患者认知和识别能力,同时提高患者的生存质量、改善DM的并发症。 |

| 【】 Objective To provide DM patients with nursing intervention services such as special education, salon activities and DM exercises to radio music by a follow-up service team composed of specialized doctors and nurses. Methods We provided follow-up care services by telephone and gave regular lectures to 231 DM patients discharged from hospital to observe their practice of blood glucose monitoring, plasma glucose control, attendance to their feet, awareness of hypoglycemia and use of insulin pen injectors. Results Of the 231 DM patients, all (100%) have been able to test their blood glucose by themselves, 209 (90.4%) have had their fasting plasma glucose back to normal, 203 have had their hemoglobin A1c (HbA1c) back to normal, 97.5% have tended to their feet in a correct way, 98.5% have been able to recognize hypoglycemia as it happens. Of the 135 patients who inject insulin with pen injectors, 125 can use them in a correct way. Conclusions Follow-up care services may promote the self-management of DM patients and thus improve their life quality. |
