| 【摘要】:目的 调查我院血透患者矿物质与骨代谢紊乱(MBD)并发疼痛的情况及其危险因素。方法 截止至2014年4月在复旦大学附属中山医院血液净化中心行血液透析治疗并符合MBD患者诊断标准的186例患者为研究对象,采用疼痛数字分级法(NRS)对患者的疼痛程度进行分级,并以Logistic回归模型分析疼痛的危险因素。结果 186例MBD患者中有99例(53.2%)存在疼痛,年龄 [OR =1.03(95% CI:1.01,1.06),P=0.01]、透析龄 [OR=1.09(95% CI:1.02,1.17),P=0.02]以及血清全段甲状旁腺激素(iPTH)水平[OR=1.01(95% CI:1.01,1.02),P=0.03]是疼痛的独立危险因素。结论 年龄、透析龄、iPTH是血透患者MBD并发疼痛的危险因素,具有高风险的患者应加强钙磷的管理,避免疾病进展,从而提高其生活质量。 |

| Abstract: Purpose To investigate the situation of mineral metabolism disorders (MBD) with pain among hemodialysis patients in our hospital. Methods By April 2014, at Blood purification center, Zhongshan Hospital, 186 patients undergoing HD treatment meet the diagnostic criteria for MBD Their degree of pain was graded using NRS, and risk factors were analysed using Logistic regression. Results 99 of 186 patients diagnosed with MBD have pain (53.2%). Age[OR =1.03(95% CI:1.01,1.06),P=0.01], dialysis age[OR=1.09(95% CI:1.02,1.17),P=0.02], iPTH[OR=1.01(95% CI:1.01,1.02),P=0.03] are independent risk factors for pain. Conclusion Age, dialysis age, iPTH are risk factors of pain in hemodialysis patients with MBD, calcium and phosphorus management should be strengthened among patients of high risk to prevent disease progression, thus improving their quality of life. |
