| 目的 探讨多学科协同的个案管理模式在糖尿病足患者中的应用效果。方法 采用便利抽样法,选取2019年1月至2020年12月在上海市第八人民医院就诊的60例糖尿病足患者,采用简单随机法分组,分为观察组(n=30),对照组(n=30),对照组给予常规换药治疗、口头健康教育及门诊随访,观察组实施多学科个案管理模式,建立糖尿病患者个案管理手册,当糖尿病足患者伤口愈合或终止接受伤口护理后随访6个月,比较两组患者在糖尿病管理自我效能、伤口愈合、血糖控制等方面情况。结果 多学科联合个案管理模式实施后,观察组患者自我效能在随访6个月后均高于对照组(P<0.05)；观察组糖尿病足伤口愈合率高于对照组组(P<0.05)；观察组患者血糖控制方面优于对照组组(P<0.05)。结论 对糖尿足患者实施多学科协同个案管理模式,可提高糖尿病足患者自我效能、糖尿病足伤口愈合及血糖控制,值得临床推广。 |

| Objective To explore the effect of applying a case management model based on specialist nurses as the core, combined with multidisciplinary collaboration, in patients with diabetic foot. Methods Convenience sampling was used to select 60 diabetic foot patients who were treated in Shanghai Eighth People"s Hospital from January 2019 to December 2020. They were divided into observation group (n=30) and control group (n=30) by simple randomization. =30). The control group was given conventional dressing change treatment, oral health education and outpatient follow-up. The observation group implemented a multidisciplinary case management model, established a case management manual for diabetic patients, and followed up 6 patients when the wound of diabetic foot patients healed or stopped receiving wound care Month, compare the two groups of patients in diabetes management self-efficacy, wound healing, blood sugar control and other aspects. Results After the implementation of the multidisciplinary combined case management model, the self-efficacy of the observation group was higher than that of the control group after 6 months of follow-up (P<0.05); the wound healing rate of the diabetic foot in the observation group was higher than that of the control group (P<0.05); The observation group was better than the control group in blood glucose control (P<0.05). Conclusion The implementation of a multidisciplinary joint case management model for diabetic foot patients can improve self-efficacy, diabetic foot wound healing and blood sugar control in diabetic foot patients, and is worthy of clinical promotion. |
