| 目的 基于eCASH理念ICU机械通气患者管理方案应用的效果评价。方法 根据患者的入院时间，将2019年5月1日-2019年9月30日入住的42组患者及其家庭设为对照组，使用目前监护室现行的医疗护理模式。将2019年10月1日-2020年2月28日入住的35组患者及其家庭设为干预组，实施基于eCASH理念ICU成人机械通气患者中的管理方案。结果 对照组患者舒适量表得分（拔管后、转出ICU时）均低于干预组舒适量表得分（P＜0.05），对照组轻度疼痛及达到理想镇静状态的频率均显著低于干预组（P＜0.05）；对照组患者和干预组患者意识模糊评分（CAM-ICU）、医学研究委员会评分（MRC-score）得分，差异无统计学意义（P＞0.05）。结论 本研究通过方案的临床实施，提高ICU机械通气患者的舒适度，为eCASH理念应用于临床实施提供一定的参考。 |

| Objective To evaluate the effect of ICU mechanical ventilation management scheme based on ecash concept. Methods according to the admission time of patients, 42 groups of patients and their families admitted from May 1, 2019 to September 30, 2019 were set as the control group, and the current medical care mode of ICU was used. 35 groups of patients and their families admitted from October 1, 2019 to February 28, 2020 were set as intervention group, and the management scheme of ICU adult patients with mechanical ventilation based on ecash concept was implemented.Result The scores of Comfort Scale (after extubation and when turning out), mild pain and ideal sedation in the control group were significantly lower than those in the intervention group (P < 0.05); there was no significant difference in the scores of CAM-ICU and MRC score in the control group (P > 0.05). Conclusion through the clinical implementation of the program, this study can improve the comfort of ICU patients with mechanical ventilation, and provide a certain reference for the application of ecash concept in clinical implementation. |
