| 摘要 目的 构建“医院-社区”联动模式下社区护士老年护理能力提升方案，以提高区域内社区老年人照护能力。 方法 基于“医院-社区”联动下社区老年护理能力提升的协作网平台、以老年护理能力提升为主的优质教学资源课程包、“医院-社区”联动下协作网同质化培训机制等方面，构建“医院-社区”联动模式下社区护士老年护理能力提升方案。实施前后对社区护士老年护理服务能力、专科护理能力、老年患者疾病管理能力进行比较。结果 “医院-社区”联动模式下社区护士老年护理能力提升方案实施后，100名社区护士老年护理核心能力与专科护理能力得到明显改善，差异具有统计学意义（P<0.05）；老年患者自我管理行为明显改善，差异具有统计学意义（P<0.05）。结论 “医院-社区”联动模式下社区护士老年护理能力提升方案能够切实提升社区护士老年护理能力，具有一定的有效性与可行性。下一步将结合国家政策导向，多层次系统性开展针对性培训，进一步提升社区护士老年护理能力。 |

| Title: the construction and application of a scheme for improving the elderly nursing ability of community nurses under the linkage mode of "hospital-community"
Abstract: Objective To improve the elderly nursing ability in the regional community,a scheme for improving the elderly nursing ability of community nurses under the linkage mode of "hospital-community" was constructed. Methods Based on the collaborative network platform for the improvement of community geriatric nursing ability under the "hospital-community" linkage, the high-quality teaching resource curriculum package focusing on the improvement of elderly nursing ability, and the homogeneous training mechanism of the collaborative network under the "hospital-community" linkage, a scheme for the improvement of community nurses' geriatric nursing ability under the "hospital-community" linkage mode was constructed. Before and after the implementation, the community nurses' geriatric nursing service ability, specialized nursing ability, disease management ability of elderly patients were compared. Results After the implementation of the geriatric nursing ability enhancement program of community nurses under the "hospital-community" linkage mode, the core competence and specialized nursing ability of 100 community nurses were significantly improved, and the difference was statistically significant (P<0.05). Self-management behavior for the elderly patients was significantly improved in elderly patients, and the difference was statistically significant (P< 0.05). Conclusion The program of improving the geriatric nursing ability of community nurses under the linkage mode of "hospital-community" can effectively improve the geriatric nursing ability of community nurses, and has certain effectiveness and feasibility. In the next step, targeted training will be carried out in a multi-level and systematic manner according to the guidance of national policies to further improve the ability of community nurses to care for the aged. |
