| 目的：探讨脑卒中吞咽功能障碍患者照顾者负担及影响因素，并对照顾者进食知识与技能需求进行分析。方法：采用自制问卷对220名脑卒中吞咽功能障碍者患者的主要照顾者进行调查，问卷包括一般情况调查表、照顾者负担量表和脑卒中吞咽障碍患者照顾者进食知识与技能需求问卷三大部分，分析其照顾负担、影响因素及进食知识与技能需求。结果：脑卒中吞咽功能障碍患者照顾者总负担处于中度水平,照护者负担水平与照顾者年龄、文化程度、经济负担、总照顾时间及日均照顾时长之间存在线性关系。脑卒中吞咽功能障碍患者进食知识与技能需求问卷总分为217分，照顾者对进食知识与技能需求较高，维度由高至低依次为经口进食知识、吞咽训练方法、管饲知识、吞咽障碍知识、营养知识、并发症护理。结论：脑卒中吞咽功能障碍患者照顾者负担处于中度水平，与照顾者年龄、文化程度、经济负担、总照顾时间及日均照顾时长密切相关，且照顾者对进食知识与技能需求较高,需根据照顾者的需求为其提供针对性的健康宣教，采取合适的进食管理干预措施，加快院外延续护理服务的发展，以减轻照顾负担，最终能达到提升照护能力、促进患者康复的目的。 |

| Analysis of caregiver burden and dietary knowledge and skill demand of stroke patients with swallowing dysfunction
Objective: To investigate the caregiver burden of stroke patients with swallowing dysfunction and its influencing factors, and to analyze the caregiver's eating knowledge and skill needs. Methods: The main caregivers of 220 stroke patients with dysphagia were investigated with self-made questionnaires, which included general situation questionnaire, caregiver burden scale and questionnaire on dietary knowledge and skill needs of caregivers of stroke patients with dysphagia. The caring burden, influencing factors and dietary knowledge and skill needs were analyzed. Results: The total caregiver burden of stroke patients with swallowing dysfunction was at a moderate level, and there was a linear relationship between caregiver burden level and caregiver age, education level, economic burden, total care time and average daily care time. The total score of the questionnaire on eating knowledge and skill needs of stroke patients with swallowing dysfunction was 217, and the caregiver had a high demand for eating knowledge and skill. The dimensions from high to low were oral eating knowledge, swallowing training methods, tube feeding knowledge, swallowing disorder knowledge, nutrition knowledge, and complication nursing. Conclusion: The caregiver burden of stroke patients with swallowing dysfunction is at a moderate level, which is closely related to the caregiver's age, education level, economic burden, total care time and average daily care time, and the caregiver has a high demand for eating knowledge and skills, so it is necessary to provide targeted health education and appropriate eating management interventions according to the needs of the caregiver. Accelerate the development of out-of-hospital extended nursing services to reduce the burden of care, and ultimately achieve the purpose of improving the nursing capacity and promoting the rehabilitation of patients. |
