| 目的 构建脑卒中患者出院计划方案,为提高合并危险因素的脑卒中患者出院质量提供参考依据。方法 基于循证方法,检索数据库,获取与脑卒中患者出院计划相关文献并进行文献质量评价,结合临床实践,采用小组讨论方法制定脑卒中患者出院计划草案,通过2轮德尔菲专家函询对草案进行修订确定最终方案。 结果 2轮专家函询中,专家积极系数均为100%,专家权威系数为 0.854、0.892，肯德尔和谐系数分别为0.235、0.485,形成包括出院评估、出院准备、出院指导、出院随访、效果评价5个方面共44个条目的脑卒中患者出院计划方案。 结论 脑卒中患者出院计划方案构建具有科学性、可靠性,可为脑卒中患者的全流程管理提供参考与借鉴。 |

| Objective To construct the discharge plan for stroke patients, and to provide reference for improving the discharge quality of stroke patients with risk factors. Methods Based on the evidence-based method, the database was searched, and the relevant literature about discharge planning for stroke patients was obtained and evaluated, through two rounds of Delphi expert letters to revise the draft to determine the final programme. Results The positive coefficient of experts was 100% , the authority coefficient of experts was 0.854、0.892, and the harmony coefficient of Kendal was 0.235,0.485, 44 items including discharge evaluation, discharge preparation, discharge guidance, discharge follow-up and effect evaluation were formed. Conclusion The establishment of discharge plan for patients with stroke is scientific and reliable, which can provide reference for the whole process management of high-risk patients with stroke. |
