| 目的 了解甲状腺肿瘤患者疾病获益感水平的现状并分析其影响因素。方法 采用方便抽样法进行横断面研究,选取2022年10月—12月在上海交通大学医学院附属瑞金医院甲状腺血管外科住院进行手术治疗的334名甲状腺肿瘤患者,使用一般资料调查问卷、中文版疾病获益感量表,社会支持评定量表,医学应对方式问卷进行调查。将单因素分析中有统计学意义的因子纳入分位数回归分析探寻疾病获益感的影响因素。 结果 甲状腺肿瘤患者疾病获益感总得分为89.00(75.00,102.00)分。分位数回归发现,社会支持在疾病获益感得分的P50、P75、P90上对其有正向影响,对P75的影响强度最大(β=0.469,P<0.05);既往史在疾病获益感得分的P50、P75上是影响因素,对P50的影响强度最大(β=-4.601,P<0.05);医学应对方式面对在疾病获益感得分的P25及以上所有分位点对其有正向影响,对P50的影响强度最大(β=1.626,P<0.05);医学应对方式回避在疾病获益感得分的P75及以下所有分位点对其有正向影响,对P10的影响强度最大(β=2.841,P<0.05);医学应对方式屈服在疾病获益感得分的P25及以上所有分位点对其有负向影响,对P25的影响强度最大(β=-2.304,P<0.05)。结论 甲状腺肿瘤患者平均拥有较高水平的疾病获益感,医护人员可根据患者疾病获益感得分的具体情况,从医学应对方式、既往史和社会支持三个方面制定个性化的干预方案,精准促进患者身心健康。 |

| Objective To understand the current situation of the level of benefit finding in patients with thyroid neoplasms and identify its influencing factors. Methods 334 patients with thyroid neoplasms who were hospitalized in a tertiary hospital in Shanghai for surgical treatment from October 2022 to December 2022 were conveniently sampled. The general information questionnaire, the Chinese Benefit Finding Scale, the Social Support Revalued Scale and the Medical Coping Mode Questionnaire were used for evaluation. Quantile regression analysis was used to explore the influencing factors of benefit finding. Results The total score of benefit finding in patients with thyroid neoplasms was 89.00 (75.00, 102.00). Quantile regression showed that social support had a positive impact on benefit finding scores at P50, P75, and P90, with the strongest impact on P75 (β=0.469, P<0.05); Past medical history had a positive impact on benefit finding scores at P50 and P75, with the strongest impact on P50 (β=-4.601, P<0.05); Confrontation had a positive impact on the P25 and above percentiles of benefit finding scores, with the strongest impact on P50 (β=1.626, P<0.05); Avoidance had a positive impact on benefit finding scores at all percentiles below P75, with the strongest impact on P10 (β=2.841, P<0.05); Acceptance had a negative impact on benefit finding scores at all percentiles below P25, with the strongest impact on P25 (β=-2.304, P<0.05). Conclusion There existed a high level of benefit finding on average in the patients with thyroid neoplasms. Healthcare professionals can promote their physical and mental health by providing personalized interventions to patients with different levels of benefit finding from medical copy modes, past medical history, and social support. |
