| 探讨元认知疗法对食管癌化疗患者元认知水平及癌症复发恐惧的影响。方法:采用随机便利法,选取2022年9月至2023年12月间医院收治食管癌化疗患者100例为研究对象,按照入院先后顺序采用随机数字表法分为对照组(n=50)与干预组(n=50),对照组采用常规护理,干预组采用元认知疗法,分别于干预前及干预第2、4个疗程末比较两组患者元认知水平、癌症复发恐惧、医院焦虑抑郁水平。结果:经重复方差分析显示:两组元认知各维度、疾病复发性恐惧、疾病进展性恐惧、焦虑、抑郁在时间、组间、交互效应差异有统计学意义(P<0.05);进一步两两比较:干预第2、4个疗程末两组元认知各维度低于干预前(P<0.05),干预组前元认知各维度低于对照组,差异有统计学意义(P<0.05);干预第2、4个疗程末两组疾病复发性恐惧、疾病进展性恐惧低于干预前(P<0.05),干预组前疾病复发性恐惧、疾病进展性恐惧低于对照组,差异有统计学意义(P<0.05);干预第2、4个疗程末两组焦虑、抑郁低于干预前(P<0.05),干预组前焦虑、抑郁低于对照组(P<0.05)。结论:食管癌化疗患者实施元认知疗法,可提高元认知水平,减轻癌症复发恐惧与负面情绪。 |

| Objective: Exploring the impact of metacognitive therapy on the metacognitive levels and cancer recurrence fear of patients undergoing chemotherapy for oesophageal cancer. Methods: Using a random convenience sampling method, 100 chemotherapy patients diagnosed with esophageal cancer admitted between September 2022 and December 2023 were selected as research subjects. They were randomly assigned into a control group (n=50) and an intervention group (n=50) according to the order of their admission, employing a random number table. The control group received standard nursing care, while the intervention group was treated with metacognitive therapy. Comparisons of metacognitive levels, fear of cancer recurrence, and levels of anxiety and depression in the hospital were conducted before the intervention, and at the end of the second and fourth treatment cycles for both groups. Results: Repeated analysis of variance revealed statistically significant differences in the dimensions of metacognition, fear of disease recurrence, fear of disease progression, anxiety, and depression between the two groups over time, as well as in interactions between groups (P < 0.05). Further pairwise comparisons showed that at the end of the second and fourth intervention cycles, the dimensions of metacognition in both groups were lower than before the intervention (P < 0.05). The metacognition dimensions in the intervention group were also lower than those in the control group before the intervention, with significant differences (P < 0.05). Additionally, at the end of the second and fourth intervention cycles, both groups exhibited lower levels of fear of disease recurrence and fear of disease progression compared to before the intervention (P < 0.05), and the intervention group had significantly lower levels of these fears than the control group prior to the intervention (P < 0.05). Lastly, anxiety and depression levels in both groups were lower at the end of the second and fourth intervention cycles compared to before the intervention (P < 0.05), with the intervention group showing significantly lower levels of anxiety and depression than the control group before the intervention (P < 0.05).Conclusion: Metacognitive therapy can improve the level of metacognition and alleviate the fear of cancer recurrence and negative emotions in esophageal cancer patients undergoing chemotherapy. |
