| 目的 探讨钝性塞丁格技术降低PICC置管及导管留置期间相关并发症的临床价值。方法 按随机数表对PICC置管患者随机分组,观察组采用钝性塞丁格技术,对照组采用常规塞丁格技术。比较两组患者术中疼痛及术后疼痛、渗血、渗液等情况。结果 2023年1月至2023年11月期间,共有164例PICC置管患者纳入本次研究,观察组和对照组各82例。观察组与对照组在性别、年龄、原发肿瘤来源、穿刺手臂、穿刺血管等方面差异无统计学意义(P均＞0.05)；所有患者均成功置管,观察组一次性送鞘成功率为98.8%,高于对照组97.6%,两组比较差异无统计学意义(χ2＝0.346,P＝0.556)；观察组术中疼痛程度轻于对照组,导管留置期间穿刺点疼痛、渗血、渗液发生率也低于对照组,两组比较差异均有统计学意义(P均＜0.05)。结论 采用钝性塞丁格技术进行PICC置管可明显减轻患者术中疼痛,减少导管留置期间穿刺点疼痛、渗血、渗液发生率,值得临床推广应用。 |

| Objective To explore the clinical value of blunt Seldinger technique in reducing complications related to PICC catheterization and early catheter indwelling. Methods Patients with PICC catheters were randomly divided into groups according to the random number table. The observation group used the blunt Seldinger technique, and the control group used the conventional Seldinger technique. The intraoperative pain and postoperative pain, bleeding, exudation, etc. of the two groups of patients were compared. Results Between January 2023 and November 2023, a total of 164 patients with PICC catheterization were included in this study, with 82 patients in the observation group and 82 patients in the control group. There was no statistically significant difference (P>0.05) between the observation group and the control group in terms of gender, age, primary tumor source, puncture arm, puncture vessel, etc; All patients were successfully catheterized, and the success rate of one-time sheath delivery in the observation group was 98.8%, which was higher than the control group"s 97.6%. There was no statistically significant difference between the two groups( χ2=0.346, P=0.556); The degree of intraoperative pain in the observation group was milder than that in the control group, and the incidence of pain, bleeding, and exudation at the early puncture point of catheter placement was also lower than that in the control group. The differences between the two groups were statistically significant (P<0.05).Conclusion The use of blunt Seidinger technique for PICC catheterization can significantly alleviate intraoperative pain in patients, reduce the incidence of pain, bleeding, and fluid leakage at the early puncture point of catheter placement, and is worthy of clinical promotion and application. |
