| 目的 促进基于循证的最佳实践在眼科门诊水合氯醛小儿镇静管理中的应用,提高临床护理质量。方法 遵循JBI循证护理中心的临床证据实践应用系统(JBI-PACES)的标准程序,通过对小儿镇静管理的相关主题的系统检索及质量评价,总结24条最佳证据,并结合临床情景和专业判断,制订15条质量审查指标,逐条审查并进行障碍因素分析,制定行动策略。实践前后分别纳入了22名护士、78名患者和22名护士、129名患儿作为研究对象。比较最佳证据应用前后眼科门诊护士审查指标的执行率、镇静成功率、镇静时间、检查配合度得分、二次给药率和不良反应发生率的差异。结果 15条审查指标中,13条审查指标的完成率均得到提高,另外2条审查指标在项目实施前执行率已达到100%；证据应用前后,镇静成功率和检查配合度显著提高(P＜0.05),镇静时间显著缩短(P＜0.05),二次给药率和镇静后不良反应发生率显著降低(P＜0.05)。结论 门诊患儿水合氯醛镇静管理的最佳证据应用可规范护士的镇静行为,改善镇静效果,确保用药安全。 |

| Objective To promote the application of best practices based on evidence in the management of pediatric sedation with chloral hydrate in ophthalmology outpatient clinics, thereby improving the quality of clinical nursing care. Methods Following the standard procedures of the Joanna Briggs Institute"s Practical Application of Clinical Evidence System (JBI-PACES), a systematic search and quality assessment of topics related to pediatric sedation management were conducted. A total of 24 best evidences were summarized. Combined with clinical scenarios and professional judgment, 15 quality review indicators were formulated. Each indicator was reviewed and analyzed for barriers, and action strategies were developed. Before and after the practice, 22 nurses, 78 patients, and another 22 nurses, 129 pediatric patients were included as research subjects. The differences in the execution rate of the review indicators, sedation success rate, sedation time, cooperation score in the examination, secondary drug administration rate, and adverse reaction incidence before and after the application of the best evidence were compared. Results Among the 15 review indicators, the completion rate of 13 indicators was improved, and the other 2 indicators had already reached a 100% execution rate before the project was implemented. Before and after the application of the evidence, the sedation success rate and examination cooperation score significantly increased (P<0.05), the sedation time significantly shortened (P<0.05), and the secondary drug administration rate and adverse reaction incidence after sedation significantly decreased (P<0.05). Conclusions The application of the best evidence in the management of chloral hydrate sedation for outpatient pediatric patients can standardize the sedation behavior of nurses, improve the sedation effect, and ensure the safety of medication use. |
