| 目的 构建适合维持性血液透析患者动静脉内瘘穿刺技术管理策略,用于指导临床护理人员动静脉内瘘规范穿刺,全方位规划患者血管通路的全生命周期,有效延长内瘘的使用寿命。方法 以JBI循证卫生保健模式为理论基础,检索国内外数据库中关于动静脉内瘘穿刺管理的原始文献,对证据进行等级及质量评价,初拟动静脉内瘘穿刺技术管理策略,然后采用德尔菲法咨询问卷的形式制定函询问卷,通过两轮的专家咨询对初步拟定的管理方案进行修正,最终形成动静脉内瘘穿刺技术管理策略。结果 共纳入15篇文献,通过循证和德尔菲法构建了“穿刺分级”、“功能评估”、“穿刺方案制定”、“并发症处理”、“患者教育”5个维度,共28项动静脉内瘘穿刺技术管理策略。结论 基于多视角决策构建的动静脉内瘘穿刺技术管理策略,构建过程严谨,方法科学,能够为透析患者动静脉内瘘穿刺技术管理提供指导。 |

| Objective To build a management strategy suitable for arteriovenousfistula puncture in dialysis patients in China, which is used to guide internal fistula puncture and prevent the incidence of complications of internal impotence.Methods Retrieve the original literature on the management of arteriovenous fistula puncture in dialysis patients in the database at home and abroad, take the JBI evidence-based health care model as the theoretical basis,grade and quality evaluation of the evidence,initially formulate the management strategy of arteriovenous fistula puncture technology,and then formulate a letter inquiry paper in the form of Delphi method consultation questionnaire,and revise the preliminary management plan through two rounds of expert consultation,and finally form a management strategy of arteriovenous fistula puncture technology.Results 15 articles were included in the study, and 28 arteriovenous fistula puncture technology management strategies were constructed through evidence-based and Delphi methods:"puncture ability classification", "functional evaluation","puncture plan formulation", "complication treatment" and "patient education".ConclusionBased on the evidence-based and Delphi method,the management strategy of arteriovenous fistula puncture technology is rigorous in construction process and scientific,which can provide guidance for the management of arteriovenous fistula in dialysis patients. |
