| 目的 了解ICU护士对疼痛管理循证实践的看法,为促进高质量的ICU疼痛管理实践提供参考依据。方法 运用现象学研究法,对贵阳市某三级甲等医院14名ICU护士进行半结构化访谈,Colaizzi7步分析法对资料进行编码、分类和提炼主题。结果 共提炼出以下6个主题及16个亚主题:参与性谈话(疼痛管理的重要性、实施困境、工作量)、行为自信(知识缺乏、疼痛共情)、物质环境改变(设备和工具、实施环境)、情感转换(行为自我调节、自主决策有限、满足感)、行为实践改变(落实行为、行为监督与反馈)、社会环境改变(疼痛管理组织建设、制度和规范、疼痛管理氛围、支持资源)。结论 ICU护士普遍持有积极的疼痛管理态度,并渴望在实践中发挥更大的主动作用,但在疼痛管理循证实践过程中,阻碍影响因素较多。提示护理管理者需要重视并优化疼痛管理制度,加强护士在疼痛管理方面的培训和能力提升,以促进ICU护士的疼痛管理循证护理实践。 |

| Objective: To understand ICU nurses" perceptions of evidence-based practice in pain management and to provide a reference basis for promoting high-quality ICU pain management practice. Methods: Using the phenomenological research method, semi-structured interviews were conducted with 14 nurses in the ICU of a tertiary hospital in Guiyang City, and Colaizzi"s 7-step analysis method was used to code, categorize, and refine the data into themes. Results: The following 6 themes and 11 sub-themes were refined: participatory talk (importance of pain management, implementation dilemmas, workload), behavioral assertiveness (lack of knowledge, pain empathy), physical environment change (equipment and tools, implementation environment), affective transformation (behavioral self-regulation, limited autonomy in decision-making, satisfaction), behavioral practice change (implementation of behaviors, behavioral monitoring and feedback), and social environment change (pain management organizational development, systems and norms, pain management climate, support resources). Conclusion: ICU nurses generally held positive attitudes toward pain management and aspired to a more proactive role in practice, but there were more influencing factors in practice. It is suggested that nursing administrators need to pay attention to and optimize the pain management system, and enhance nurses" training and competence in pain management in order to promote evidence-based nursing practice of pain management among ICU nurses. |
