| 目的 探究脑卒中后睡眠障碍潜在睡眠分型,分析不同睡眠分型的影响因素。 方法 采用便利抽样法,选取2024年1月—2024年8月在南通市某三级医院住院的脑卒中患者为调查对象。使用一般资料调查表、匹兹堡睡眠质量指数(PSQI)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、简易智力状态检查量表(MMSE)进行调查。根据PSQI各维度得分对脑卒中患者睡眠分型进行潜在剖面分析,采用无序多分类 Logistic 回归分析探究脑卒中后睡眠障碍的影响因素。 结果 共纳入290例脑卒中患者,潜在剖面分析可分为3个潜在类别:睡眠相对良好型(52.1%)、睡眠障碍-睡眠不足型(34.8%)、严重睡眠障碍-药物催眠型(13.1%)。其影响因素包括性别、脑卒中次数、脑卒中部位、脑卒中时期、SAS得分、SDS得分以及MMSE得分(P<0. 05)。 结论 脑卒中患者睡眠情况存在明显的群体异质性,应多关注女性、脑卒中次数大于3次、病灶部位为多部位、脑卒中时期处于急性期、有焦虑、抑郁倾向以及存在一定认知障碍的脑卒中患者睡眠状况,可针对不同类别特征实施个体化干预,以促进脑卒中患者神经功能恢复,改善生活质量,降低脑卒中后的致残率和复发风险。 |

| Objective To explore the potential sleep typing of post-stroke sleep disorders and analyse the influencing factors of different sleep typing. Methods Stroke patients admitted to a tertiary hospital in Nantong City from January 2024 to August 2024 were selected for the survey using convenience sampling method. A general information questionnaire, Pittsburgh Sleep Quality Index (PSQI), Self-Assessment Scale for Anxiety (SAS), Self-Assessment Scale for Depression (SDS), and Brief Mental Status Examination (MMSE) were used to conduct the survey. Potential profiles of sleep typing in stroke patients were analysed according to the scores of PSQI dimensions, and unordered multicategorical Logistic regression analysis was used to explore the influencing factors of sleep disorders after stroke. Results A total of 290 stroke patients were included, and the potential profiling analysis could be classified into 3 potential categories: relatively good sleep type (52.1%), sleep disorder-sleep deprivation type (34.8%), and severe sleep disorder-medicated hypnosis type (13.1%). The influencing factors included gender, number of strokes, stroke site, stroke period, SAS score, SDS score, and MMSE score (P<0. 05). Conclusion There is obvious group heterogeneity in the sleep status of stroke patients, and more attention should be paid to the sleep status of stroke patients who are female, have more than 3 strokes, have multi-site foci, are in the acute phase of stroke, have a tendency of anxiety and depression, and have certain cognitive impairment.
