      摘要 目的 系统评价妇科癌症患者经济毒性现状及影响因素。方法 系统检索PubMed、Web of Science、EMbase、Cochrane Library、知网、万方和维普数据库,搜集有关妇科癌症患者经济毒性现状和(或)影响因素的研究,检索时限为建库至2025年4月,同时检索纳入研究的参考文献,进一步获得有关资料。采用美国卫生保健质量和研究机构(Agency for Healthcare Research and Quality,AHRQ)横断面研究偏倚风险评价标准对纳入研究进行评价。采用Stata17.0软件对经济毒性发生率进行Meta分析,对经济毒性的影响因素采用描述性分析。结果 最终纳入11篇文献,涉及2165例研究对象。Meta分析结果显示:妇科癌症患者经济毒性发生率为49%(95%CI:38%~60%),经济毒性得分19.39(95%CI:11.91~26.87)。年龄、职业状况、医疗保险、收入及存款、社会支持、治疗方案、肤色、恐惧疾病进展、置管方式和癌症类型可能为妇科癌症患者经济毒性的影响因素。结论 妇科癌症患者经济毒性现状不容乐观,并受多元因素影响,建议根据相关影响因素早期筛查高危人群,并制定相应干预措施行针对性预防。

      Abstract Objective To systematically evaluate the current status of financial toxicity and its influencing factors in patients with gynecological cancer. Methods A systematic search was conducted in PubMed, Web of Science, EMbase, Cochrane Library, CNKI, Wanfang, and VIP databases for studies on the current status of financial toxicity and/or its influencing factors in patients with gynecological cancer. The search period was from the establishment of the databases to February 2025. References of included studies were also searched to obtain additional relevant data. The Agency for Healthcare Research and Quality (AHRQ) cross-sectional study bias risk assessment criteria were used to evaluate the included studies. Meta-analysis was performed using Stata 17.0 software. Results A total of 11 studies involving 2,165 subjects were included. The meta-analysis results showed that the incidence of financial toxicity in patients with gynecological cancer was 49% (95% CI: 38%~60%), and the financial toxicity score was 19.39 (95% CI: 11.91~26.87). Age, occupational status, medical insurance, income and savings, social support, treatment plan, skin color, fear of disease progression, catheterization method, and cancer type may be influencing factors for the incidence of financial toxicity in patients with gynecological cancer. Conclusion The current situation of financial toxicity in gynecological cancer patients is not optimistic. It is recommended to screen high-risk groups early based on relevant influencing factors and formulate corresponding intervention measures for targeted prevention.
