目的:调查急性胰腺炎患者代理决策者决策困境现状,并分析其影响因素。方法:采用便利抽样法,选取某三级甲等医院收治的286例急性胰腺炎患者的代理决策者为研究对象,采用一般资料调查表、决策困境量表、状态焦虑量表、决策疲劳量表、感知社会支持评定量表进行问卷调查。结果:急性胰腺炎患者代理决策者决策困境总分为(32.58±6.73)分,其中75.17%的代理决策者处于中度及以上决策困境。多元线性回归分析显示,患者病情严重程度、近一年患者经急诊入院次数、代理决策者文化程度、既往有无该疾病决策经历、焦虑水平、决策疲劳、感知社会支持水平是决策困境的主要影响因素(P<0.05)。结论:急性胰腺炎患者代理决策者普遍面临中等程度的决策困境,临床医护人员应关注代理决策者的心理状态,加强疾病信息支持、结构化沟通和心理干预,以降低其决策困境水平。

【Abstract】:Objective This study aimed to investigate the current status of decision-making dilemmas among surrogate decision-makers for patients with acute pancreatitis (AP) and analyze its influencing factors. Methods A convenience sampling method was used to recruit 286 surrogate decision-makers of AP patients admitted to a tertiary care hospital. Data were collected using a general information questionnaire, the Decision Conflict Scale (DCS), State-Trait Anxiety Inventory-State (STAI-S), Decision Fatigue Scale (DFS), and Perceived Social Support Scale (PSSS). Results The total score of decision-making dilemmas among the surrogate decision-makers was (32.58 ± 6.73). Of these, 75.17% reported moderate or higher levels of decision-making dilemmas. Multivariate linear regression analysis showed that the severity of the patient’s condition, the number of emergency admissions in the past year, the educational level of the surrogate decision-maker, prior experience in decision-making for AP, anxiety level, decision fatigue, and perceived social support were significant influencing factors (P < 0.05).Conclusion Surrogate decision-makers for AP patients commonly face moderate decision-making dilemmas. Clinical healthcare providers should pay attention to the psychological state of surrogate decision-makers, strengthen disease information support, structured communication, and psychological interventions to reduce their decision-making dilemmas.
