| 【】 目的 探讨对农村2型糖尿病患者进行健康教育的新型方法。以社区一医院一体化的无缝管理模式联合集束化干预的方法对农村2型糖尿病患者进行健康教育,观察教育效果。方法 选取8个村委的152名2型糖尿病患者纳入研究,随机分为实验组和对照组各4个村委,分别为76人。两组均由同一批全科医师进行教育。对照组给予常规教育口头并发放知识宣传小册。实验组由三级医院的内分泌专家先对社区全科医师进行培训,再由该批全科医师对患者进行集束化教育。比较全科医师受培训前后的糖尿病知识测评改变情况,以及两组患者教育前后糖尿病知识水平及自我管理能力、糖化血红蛋白改变情况。结果 培训后,全科医师糖尿病知识掌握情况明显提高,t=7.02,P<0.01。受教育后实验组患者的糖尿病知识水平及自我管理能力得分提高,HbA1C下降,P <0.05。而对照组未见明显改变, P＞ 0.05。结论 社区一医院一体化的无缝管理模式联合集束化干预的方法作为一种新型的健康教育方法,对农村2型糖尿病患者教育方法可行,能提高患者糖尿病知识的知晓情况及自我管理能力,并能改善患者的糖化血红蛋白的水平,同时也可提高社区全科医师糖尿病知识的水平。 |

| Abstract ：Objective Exprole a new way to make health education for the coutries type 2 diabetes patients.The coutries type 2 diabetes patients were made health education with the way of community-hospital-integrated seamless management combined with bundles intervention.And then the effect was observed.Methods Select 152 type 2 diabetes patients coming from 8 coutries to make the reseach.All the patients were divided into experimental group and control group randomly. The experimental group had 76 patients coming from 4 coutries ,and the control group also had 76 patients coming from the else 4 coutries . The two groups patients were both educated by the same general practitioners.The control group patients were educated with the normal oral education channel.Then the general practitioners were trained by the diabetologist and the seniority diabetes education nurses coming from the tertiary hospital.The experimental group patients were educated with the bundles method by the trained general practitioners.Both the two groups patients were send DM knowledge publicity brochures.Then DM knowledge test results of the general practitioners were compared before and after training.And glycosylated hemoglobin、DM knowledge test results and self-management ability were also compared before and after education from the two groups patients. Results After the training ，the awareness of DM knowledge in the general practitioners was enhanced obviously (t=7.02，P<0.01).After the education, glycosylated hemoglobin、DM knowledge test results and self-management ability were improved in the experimental group patiens( P<0.05).While in the the control group patients there were no distinct change.Conclusion As a new way , community-hospital-integrated seamless management modle combined with bundles intervention is feasible,which can improve glycosylated hemoglobin、DM knowledge test results and self-management ability in the coutry-community type 2 diabetes patients.Otherwise the awareness of DM knowledge in the general practitioners can be also enhanced with chain training method. |
