【摘要】目的：探讨胆红素吸附术在以高胆红素血症为主要表现的重症肝炎患者中的疗效、安全性和护理特点。方法：纳入复旦大学附属华山医院2011年1月到2013年12月三年间收治的重症肝炎病例并需要行胆红素吸附治疗的共36例，治疗机器为可乐丽的KM8900，每次治疗血浆量为40ml/kg，治疗时间为3小时，一个疗程治疗2-3次。治疗前做好心理疏导及充分的准备工作，治疗中监测患者生命体征并预防并发症，治疗后做好静脉留置导管的护理，治疗前后检测常规实验室指标的变化。结果：在治疗过程中，所有患者均对胆红素吸附治疗耐受良好。治疗后患者血清总胆红素水平显著下降71.8±0.2%，丙氨酸转氨酶、血氨亦有明显下降，临床症状均有不同程度的改善。治疗前后心率、血压等生命体征平稳，余实验室指标均没有明显改变。结论：胆红素吸附技术清除胆红素效率高，对生命体征、内环境影响小，是临床上一种安全有效的治疗手段，良好优质的护理是胆红素吸附技术临床运用的重要保障。

英文摘要:

Objective. To evaluate the clinical utility effectiveness and nursing characteristic of Bilirubin Adsorption (BA) in serious hepatitis with Hyperbilirubinemia. Methods. Thirsty-six patients with hyperbilirubinemia during 2011.1 to 2013.12 in our hospital were recruited to our study. They were treated by the machine KM8900 of Kuraray, the therapy plasma volume was 40ml/kg, therapy time was 2-3 hours, and one treating course was 2-3 times. Psychological counseling, full preparation, nursing care of venous catheter and observation of vital signs were provided during the treatment. Laboratory tests were measured both before and after BA. Results. All the patients were well tolerated during the whole treatment of bilirubin adsorption. A sharp decrease of serum total bilirubin concentration was observed after treatment. ALT and blood ammonia also declined obviously and clinical symptoms were improved in different degrees. The vital signs were stable and no change was found in all the laboratory indexes after treatment.Conclustions. BA was an effective and safe treatment for hyperbilirubinemia due to its high clearance of bilirubin and little side effect on vital signs and inner environment.
