| 目的：探讨自制新型气管切开固定带的临床应用与效果。方法：选取我科2008年3月～2011年3月患者40例为对照组,2011年4月～2014年4月患者40例为实验组。实验组采用自制新型气管切开固定带固定气管切开套管,而对照组采用传统的白纱带固定,比较两组患者颈部压疮、湿疹、过敏和感染发生率、气管导管滑脱、移位以及患者舒适度等方面的情况。结果：实验组颈部压疮发生率明显低于对照组(χ2=0 VS χ2=10.14,P＜0.01)；实验组的湿疹发生率(χ2=0 VS χ2=0.213)、感染发生率(χ2=0 VS χ2=2.051)、气管切开套管移位发生率(χ2=0 VS χ2=3.117)虽尚未显著低于对照组(P＞0.05),但呈现下降现象；同样,实验组患者舒适度高于对对照组(χ2=0 VS χ2=2.797),但尚未呈现统计学差异性(P＞0.05)。结论：新型气管切开固定带能有效预防压疮的发生,提高患者舒适度,确保临床护理安全,值得在临床上推广应用。 |

| [Abstract] Objective: To discuss the effects of the newly self-made method for fastening the outer tube of tracheotomy in clinical application. Methods: In our hospital, a total of 40 patients from March 2008 to March 2011 were set as control group. And 40 patients from April 2011 to April 2014 were enrolled in the experimental group.The control group adopted the traditional method of using cotton strings, while the experimental group adopted the newly self-made method. The incidence rate of pressure ulcers, eczema, irritability and infection of the neck, the slippery of the outer tube of tracheotomy and the comfort of the patients were compared. Results: The incidence of the pressure ulcers of the neck in the experimental group was significantly lower than the control group (χ2=0 VSχ2=10.14,P<0.01); the incidence of the eczema of the neck in the experimental group (χ2=0 VSχ2=0.213)、the incidence of the infection of the neck (χ2=0 VSχ2=2.051), the incidence of the shift of the outer tube of tracheotomy (χ2=0 VSχ2=3.117) were not significantly lower than the control group (P>0.05), but it was a downward trend; Meanwhile, the comfort of the experimental group was higher than the control group (χ2=0 VSχ2=2.797), but there was no significant difference (P>0.05). Conclusion: The newly self-made method for fastening the outer tube of tracheotomy can effectively prevent the incidence rate of pressure ulcers, improved comfort of the patients and ensure the nursing safety in clinical practice. Thus, the new method is worth to be popularized in clinical practice. |
