      目的 探讨新型气管切开堵管帽在气管切开患者堵管过程中的安全性。方法 自制新型气管切开堵管帽,并应用于临床实践；回顾性分析新型气管切开堵管帽实施前后各1年内所有入住本院急重症医学科行气管切开并经积极治疗后成功拔除气管套管患者资料,比较两组患者性别、年龄、基础疾病、患者舒适度、意外事件发生率和一次性堵管成功率等。结果 两组患者性别、年龄、基础疾病和意识状态差异无统计学意义；两组中最多见的基础疾病均为脑肿瘤术后患者和重症肺炎/急性呼吸窘迫综合症患者；与对照组比较,观察组新型气管切开堵管帽堵管方法能显著增加患者堵管过程中的舒适度,降低意外事件的发生率(P＜0.05)。结论 运用新型气管切开堵管帽对气管切开患者进行堵管能显著增加患者堵管过程的舒适度,降低不安全事件发生率,保障护理安全。
英文摘要:
      Objective To explore the safety of patients with tracheostomy in the process of plugging with new tracheotomy plugging cap. Methods A new tracheotomy plugging cap was designed and used in clinical practice. The information of patients with tracheostomy in emergency-critical care medicine department patients between one year before and after the implementation of new tracheotomy cap were retrospective analysed. The age, sex, underlying disease, the comfort degree of patients, the incidence of accidents and the success rate of one-time plugging were compared between two groups. Results There were no significant differences in sex, age, underlying disease and glasgow coma scale(GCS) between the two groups. The most common underlying diseases in both groups were post-operative patients with brain tumors and patients with severe pneumonia / acute respiratory distress syndrome. Compared with the control group, utilizing the new tracheostomy cap can significantly increase the comfort degree of patients in the process of plugging and reduce the incidence of the accident in the observation group (P <0.05). Conclusion The utilizing of the new tracheostomy plugging cap can effectively increase the comfort degree of patients with tracheal plugging, reduce the incidence of unsafe incidents and ensure the safety of nursing.
