      【】目的：探讨结合社区联动的延续护理对提高高血压患者服药依从性和健康行为。方法：选取我院自2016年8月~2017年4月收治的120例原发性高血压患者作为研究对象,随即分为对照组和观察组,各60例。对照组患者实行常规护理模式,观察组患者在常规护理的基础上进行结合社区联动的延续性护理。随访问卷调查分别在患者就诊时、诊疗后1月、3月、6月进行,指标包括高血压防治知识,服药依从性和健康行为情况。结果：观察组患者高血压防治知识、服药依从性和健康行为均明显优于对照组治疗。且差异均有统计学意义(P＜0.05)。结论：结合社区联动的延续护理能提高高血压患者高血压防治知识和服药依从性,促进健康行为,提高治疗效果。
英文摘要:
      Objective: To explore the effect of community-based post-discharge continuing care on drug compliance and health behavior in hypertensive patients. Methods: 120 patients with essential hypertension in our hospital from August 2016 to April 2017 were regarded as the research objectives. They were randomly divided into the control group and the observation group, with 60 patients per group. Patients in the control group received routine nursing care, while those in the observation group received continuing nursing care based on the routine nursing care from the community. Both of groups were assessed with the indexes such as knowledge of hypertension prevention, drug compliance and health behavior by the follow-up questionnaires given out at the time of seeing a doctor, 1, 3 and 6 months later. Results: The indexes of knowledge of hypertension prevention, drug compliance and health behavior in the observation group were significantly superior to those of control group with statistical difference (P < 0.05). Conclusion: The post-discharge continuing care from community can improve the knowledge of prevention, treatment and drug compliance of hypertension and promote health behavior and therapeutic effect.
