目的总结65例重症高胆红素血症患儿实施全自动外周动静脉同步换血疗法的护理经验。方法收集2015年1月至2016年12月在我院新生儿重症监护病房采用外周动静脉同步换血疗法进行高胆红素血症患儿治疗的临床资料并对其疗效、并发症及护理进行分析总结。结果 65例患儿100%顺利完成换血,换血过程中未发生严重并发症,患儿均好转出院。结论全自动外周动静脉同步换血技术安全有效,可操作性强,换血通路的建立和维护是顺利有效换血的关键,换血过程中对患儿病情及不良反应的密切观察、维持出入量平衡则是保证换血安全的关键。

英文摘要:

Objective Summarized nursing experience of 65 cases of severe hyperbilirubinemia with full automatic peripheral arteriovenous synchronous blood exchange. Methods The clinical data of hyperbilirubinemia treated by peripheral arteriovenous synchronous exchange transfusion in our neonatal intensive care unit from January 2015 to December 2016 were collected. The effect, complication and nursing practice of peripheral arteriovenous synchronous exchange transfusion were analyzed and summarized. Results Sixty-five infants (100%) completed the blood exchange successfully. No serious complications occurred during the blood exchange process. All infants were cured or improved and discharged. Conclusion Full automatic peripheral arteriovenous synchronous blood exchange technology is safe, effective and operational. The establishment and maintenance of the vascular pathway is the key to effective blood exchange. Close observation and the maintenance of the fluid balance in the process is the key to ensure the safety of blood exchange.
